🚨#ESMO26 and last but not least Top Lung Abstract goes to the new Diva 💃 of #KRASG12C #lungcancer! #KRASCENDO1: Phase III #Divarasib vs sotorasib/adagrasib after prior chemotherapy for #KRASG12C NSCLC 📈 PFS: 9.8 vs 5.7 mo (HR 0.58) 🔥 OS: ~18 vs 12 mo (HR 0.56) 🎯 ORR: 58.4% vs 32.3% ⚠️ Grade 3–4 TRAEs: 24.6% vs 34.5% 🏆This is clearly practice changing with a much better tolerated drug and fewer side effects ( although still GI toxicity) compared to current standard for this high-risk population. 🤔Still some questions about exclusion of untreated brain mets and low number of adagrasib treated pts in control arm #lcsm @KRASKickers

#ESMO26 🇪🇸 Top Lung Abstracts 🫁 #SAFFRON Phase 3 study of #savolitinib + osimertinib vs. platinum–pemetrexed for MET- driven resistance post osimertinib (N=338) 🎯 MET IHC 3+/ Amp (FISH)>10 🎯 PFS: 7.6 vs 5.4 mo (HR 0.55) 🎯 OS: 17.8 vs 12.9 mo (HR 0.68) 🎯 ORR: 51% vs 27% ⚠️ AEs: Edema & GI toxicity 👉🏽 Clinical Challenge - Although MET resistance post osi can occur 20-30%, it is hard to test for this prospectively in clinic- requires repeat biopsy🔪 #LCSM #LungCancer @EGFRResisters

🚨 #ESMO26 | Top Lung Abstracts🫁 PERSEE: Phase 3 🇫🇷 study of pembrolizumab alone vs chemo+pembro for PD-L1-high 🔥 PFS: 16.1 vs 6.9 mo (HR 0.56) 🎯 ORR: 77.5% vs 50.3% ⚠️ Early progression: 5.8% vs 26.5% 📊 OS: 50.2 vs 29.9 mo (HR 0.77; NS) 👉🏽 Maybe more is better for select patients 👉🏽Benefit persisted even with PD-L1 ≥90%—but grade ≥3 TRAEs increased (53.8% vs 19.3%). More disease control but more toxicity. #lcsm #immunotherapy

🚨 #ESMO26 | Top Lung Abstracts 🫁 OptiTROP-Lung04 (Phase III- China only trial) Significant mOS 8.5 mo advantage for sac-TMT, a TROP2 ADC, after EGFR-TKI progression in EGFR-mutant NSCLC🫁 vs chemo! 📊 Median OS: 25.9 vs 17.4 months 🔥 HR 0.69 — 31% lower risk of death ⚠️ AEs- cytopenias, stomatitis, opthal, ILD 👉🏽Competes w Dato-DXd already approved in this EGFR+ population based on Tropion-Lung05, Lung01 👉🏽Global validation and treatment-sequencing questions remain. @EGFRResisters #LCSM #LungCancer #ESMO26

🇪🇸 #ESMO26 | Top Lung Abstracts 🫁 🚨 OPTITROP-Lung06: Phase III trial in PD-L1–negative, driver-negative nonsquamous NSCLC: 🔥 Median PFS: 15.0 vs 5.7 months 📉 HR 0.49 — 51% reduction in risk of progression/death 🎯 DOR: 13.6 vs 6.8 months 📊 12-mo OS: 77.6% vs 66.4% (immature data) ▶️ I wouldn’t call this a “chemo-free” regimen because the toxicity of an ADC is similar to chemo—> but this many be a potential option for this hard to treat PDL1 neg population. ▶️Could this change the 1L landscape? 👀maybe? but I would hope for a bigger survival advantage….data immature … #LungCancer #LCSM #ADC

🇪🇸 #ESMO26 Top Lung Abstracts | SARON 🫁 ☢️In synchronous oligometastatic, driver-negative #NSCLC, adding consolidative RT to systemic therapy improved PFS (13.9 vs 8.9 months; HR 0.64) and local control. ❗But no OS benefit (HR 0.99; p=0.95). Better disease control doesn’t always translate into longer survival! 🔬 #LungCancer #LCSM

#DeLLPhi305 Low US enrollment in this pivotal #SCLC trial is NOT surprising. Regulatory hurdles, costs & complexity push sponsors to conduct trials faster ex-US—even for drugs destined to be used at greater rates by US patients. Why it matters: ▶️ Less evidence on implementation & AEs in diverse populations ▶️ Fewer opportunities for US investigators to shape drug development early We need faster, more inclusive trials! 🔬#clinicaltrials #ESMO26 @SclcSMASHERS @GlopesMd
How is it possible that, with all these hospitals in the study actively recruiting, they contributed ONLY slightly more than ±10% of the patients for the study #DeLLphi-305? I can´t understand it #SCLC #Tarlatamab #ESMO26
