Excellent presentation by @tompowles1! VOLGA: perioperative durvalumab + 3 cycles of neoadjuvant EV improved EFS (HR 0.51) and OS (HR 0.58) vs cystectomy ± adjuvant therapy in cisplatin-ineligible/declining MIBC. ADCs moving earlier! #ESMO26 @oncoalert @myESMO

#ESMO26 Phase II sac-TMT + pembro in 1L cisplatin-ineligible advanced UC (n=40, 60% UTUC) Median PFS 11.2 mo, OS NR and G3/4 TRAEs 63% Promising results although its place in the EVP era remains to be defined. Worth exploring this combination in other settings @OncoAlert

SAC-TMT + pembro shows 68% response rates in metastastic bladder cancer #ESMO2026 . Trofuse-55 explores this combination in bladder sparing approaches in a randomized phase 3. The triplet with EVP seems a justified research randomised question. Triplet data from yesterday showed 81% RRs. @OncoAlert

Kudos @tompowles1 on a fantastic #VOLGA presidential @ #ESMO26 👏 Less is More! • Cis-inelig MIBC, 25% were cis refusers
• D+EV x3 → RC → adj D x9 (NO adj EV)
• EFS HR 0.51
• OS HR 0.58
• pCR 45 vs 10%
• G3-4 TRAE 30%,
• neuropathy only 18%
• imAEs 33% (G3-4 11%)
• GHS/QoL maintained post-RC • Treme ↑tox (G3-4 42%, 5 deaths), NO OS gain. My take: it's not just about chasing bigger numbers. Tox matters too, especially when OS is achieved. Need to discuss the pros and cons with patients. Less PN and skin tox, and fewer G3+ immune AEs than more cycles and that's a win for patients ! 🙌 @Uromigos @OncoAlert @brian_rini @LosUromigos @drenriquegrande

VOLGA also explored short periods of EV with duvalumab and tremelimumab in muscle invasive bladder cancer #ESMO26 @NEJM. Results showed EVDT improved EFS vs cystectomy (HR 0.61). Significant OS was not reached at this interim (HR 0.77 -ongoing analysis). EVDT = 10% higher rates of G3/4 toxicity and less EV delivery than the EV durvalumab arm. More is not always better. It appears the addition of treme partly compromised delivery of EV, which likely accounts for the more modest results compared to the doublet @OncoAlert

#ESMO26 KEYNOTE-B15 HRQoL results further support perioperative EVP in MIBC No clinically meaningful differences in HRQoL despite improved EFS and OS. The greatest deterioration occurred in both arms after surgery (sexual dysfunction) Congrats @Uro_Oncologist! @OncoAlert

Proud to share our exploratory analysis from DISCUS at #ESMO26 @BartsECMC - 34% of patients never reached avelumab - 3C vs 6C increased transition (74% vs 57%), with fewer PD/death events - QoL remained stable on ICI and baseline PS was the only differentiating factor @OncoAlert

ctDNA analysis from the EVP vs chemotherapy muscle invasive bladder cancer trial (B15/EV304) shows ctDNA is strongly prognostic at all time points for both arms. EVP has higher clearance rates across time points, underlining its ⬆️ activity. Results for the control arm are strikingly similar to the ctDNA results in the chemo control arm in the NIAGARA trial. This consistency underlines the utility of ctDNA from a prognostic perspective.#ESMO26 @OncoAlert

#ESMO26 Adding perioperative pembro to Gem/Cis did not significantly improve EFS in cis-eligible MIBC (HR 0.88) in KN866. Higher pCR rates (although not formally tested) were seen in the pembro arm. More info in the concomitant publication in @Annals_Oncology @OncoAlert

It hard to explain why 1 trial with gem/cis +/-PD(L)1 (NIAGARA) is +ve for EFS/OS and 2 very similar trials (presented today) are negative in MIBC. Potentially NIAGARA has the highest risk population, not all respond to IO and the maintenance phase the most effect- but the OS of gem/cis pembro HR of 1 is concerning. This reflects what we saw in metastatic disease with contradictory results for the big combination trials. Overall EVP looks better and more consistent #ESMO26 @myESMO @Annals_Oncology @OncoAlert

What happens when clinical trial results don't align? Two Phase 3 trials presented at #ESMO2026 did not demonstrate a benefit from adding immunotherapy to chemotherapy MIBC. Dr. Rob Jones joins Brian and Tom to unpack the findings, explore how they fit alongside the NIAGARA data, and discuss the implications for neoadjuvant and adjuvant treatment strategies. 🎙️ Episode 521 of The Uromigos Podcast is now live: bit.ly/4z7YGQ5 #Uromigos #BladderCancer #GUOncology
