📚 Not a novel concept. Prior work has suggested that higher intratumoral dose improves local control!
✅ What this study contributes is additional clinical evidence, plus a practical planning strategy (SPICE: Stereotactic Planning with Intentional Central Escalation).
⚠️ No PFS or OS advantage seen.

🔬 Early post-treatment molecular residual disease (MRD):
👉 MRD+ was significantly associated with inferior PFS and OS
📈 Longitudinal surveillance:
👉 24-month PFS: 20.8% vs 96.4% (MRD+ vs MRD−), regardless of p16 status.
🔺MRD positivity preceded radiographic progression by a median of 4.3 months
🎯 These findings highlight the potential of longitudinal ctDNA monitoring in HNSCC.
