It doesn't. It happens the same way the plumbing system knows that you need water in the third bathroom of your apartment when you open the tap.
The blood is constantly across the body through blood vessels. All body parts have blood vessels running through them, up too the tiniest layer of you skin in form of capillaries. The pain killers when ingested gets absorbed into the blood flow from the walls of the stomach and they keep moving around every part of the body.
The pain killers often do not address the pain in your pinky toe alone, unless that's the only place where you feel the pain. How you know this is that you often sleep better or feel better when you wake up..
