At the Menopause Society (TMS) 2026 Annual Meeting, Cheryl Cox Kinney, MD, FACOG, MSCP, describes a patient case that fundamentally changed how she approaches night sweats. Kinney notes that most women with night sweats not attributable to menopause do not have a serious underlying cause, but this case taught her to take the symptom seriously and keep a broader differential in mind. Jean Cunningham, PharmD, BCPS, urges ob-gyns to broaden their index of suspicion for perimenopause beyond the classic presentations of hot flashes and irregular periods. Early or atypical manifestations can include new-onset sleep disruption, mood changes, anxiety, irritability, brain fog, palpitations, migraines, joint aches, and worsening PMS—symptoms that may not immediately prompt a hormonal workup.
