youtu.be/vEtnMSMFI3k A difficilt case of primary PUJ obstruction with a 6 cm stenotic segment and no history of ureteral intervention. We performed a robot-assisted Culp–DeWeerd spiral flap pyeloplasty using the SSI Mantra system. With the renal pelvis distended by the PCN, we felt a 6 cm flap could reach the ureter. Buccal mucosal graft ureteroplasty was another option considered. At 3 months after surgery, the patient is off the stent and PCN, with no fever or pain. Hydronephrosis has significantly reduced. A rare paediatric case: robotic ureteral tapering and reimplantation in a 7-year-old with a solitary pelvic ectopic kidney, grade IV VUR and megaureter. At 45 days, the stent is out; the child is voiding well, infection-free, and creatinine has improved from 3 to 1.4 mg/dL.
