🇮🇹 Pancreatic cancer in Italy: what will 2035 look like?
📈 >13% increase in deaths
👴 Driven largely by population aging
⚠️ But with a striking North–South divide in mortality trends.
👉Forecasting disease burden is healthcare planning👈

😨 If you do pancreas surgery you do FEAR chronic corticosteroid therapies…
🇮🇹🇺🇸🇳🇱🇯🇵 2.5k patients matched per therapy
💊Chronic steroids led to ⬆️ morbidity, mortality (10%!) and infections
🔴 Risk stratification should be balanced accordingly!

Great collaboration with @CUpancreas 🇮🇹 🇺🇸🎓🎓🎓
How to manage chronic corticosteroid therapy before pancreatic surgery?
🔥 Increased morbidity burden and 5× higher mortality
🗣️ Preoperative counselling is essential
🤝 Multidisciplinary, risk-adapted perioperative planning

Pancreatic cancer surgery in Italy 🇮🇹outcomes by hospital volume
🏥 3.4k surgeries / 90day mortality 10%
🔴 <30 surg - mortality > 10% = 21%
🟡 30-50 surg - mortality >5% = 18%
🟢 >50 surg - mortality <5% = 61%
📣 Pancreas unit urgently needed 📣

Personalised decision-making in hepatocellular carcinoma: conceptual evolution over two decades
➡️doi.org/10.1093/bjsopen/zrag…
Evidence-based medicine was conceived to integrate best available evidence, clinical expertise, and patient values, yet in many fields it has come to be equated with rigid guideline algorithms. This narrative, conceptual review draws selectively on landmark staging frameworks, guidelines, and methodological literature to trace two decades of decision-making in hepatocellular carcinoma across a sequence of schematic frames: stage-centred algorithms, recognition of differential treatment effects, multiparametric expert deliberation, and dynamic, time-dependent bidirectional strategies reinforced by immune checkpoint inhibitor combinations. The trajectory culminates in explicit acknowledgement of irreducible uncertainty, with the multiparametric therapeutic hierarchy positioned at the interface between baseline and treatment-conditioned prognosis and aligned with GRADE evidence-to-decision principles. The authors conclude that hepatocellular carcinoma exemplifies personalised decision-making in complex multidisciplinary care, where transparency about what is known, what is uncertain, and where value judgements enter is preferable to further algorithmic refinement.

Uncertainty in medicine isn't a flaw to engineer away with finer algorithms. It's intrinsic to every individual decision.
Our new paper in BJS Open traces 20 years of liver cancer to make the case. Honoured to have Gordon Guyatt as co-author.

✋ STOP surveillance in IPMN was a hot topic 🔥 but controversial
💰 Current policies not cost effective but concomitant PDAC is a big issue in long run … more evidence needed!

What do we mean with "recurrence" of IPMN 🍇 after surgery ?!
☝️ word - ✌️ different scenario
🆕 IPMN after resection of non-invasive are "de novo" in 50% cases
🦀 Recurrence of invasive IPMN was 42%, most systemic if 🍒 node + and tubular type

Drain management after pancreatoduodenectomy: risk-stratified dynamic algorithm
➡️doi.org/10.1093/bjsopen/zrag…
This multicentre study of 788 patients undergoing pancreatoduodenectomy (PD) evaluates the reliability of drain fluid amylase (DFA) as a predictor of postoperative pancreatic fistula among high-risk (HR) and non-HR-PD. We found that, whereas early drain removal guided by DFA and serum enzymes is safe in non-HR-PD, DFA accuracy is reduced in HR-PD until postoperative day 5, when combined DFA and C-reactive protein levels allow safe drain removal. These findings support a dynamic, risk-based drain management protocol tailored to pancreatic fistula risk.
