A lot of talk of emergency department REBOA, but how many places are actually using it? Is it that widespread?
@HamzaHanifMD sought to answer that question using TQIP data from the @AmCollSurgeons with his paper hot off the press in @AmJSurgery.
Bottom line: Most centers have no documented ED use
2022 represents the most recent public dataset available, for which he found:
-195 out of 816 contributing trauma centers had documented use
-71% were level 1 centers, 29% were level 1, and <1% were level 3
-The most common post-REBOA hemorrhage control surgery was to the pelvis
A few caveats to note:
-Many centers were missing verification levels, so it may be underestimating some use
-Many procedures were missing procedure times, which limits our ability to sort out which ones occurred in the ED versus other locations, so they were excluded
-Underdocumentation is always a challenge within the registry

Once you have vascular control which is the hard part, vascular suturing on the robot is easier and more fun than open as you have 9x magnification, articulated instruments, no tremor, and you’re sitting comfortably 😅

Our recent NIS database analysis showed higher mortality risk among females and lower mortality risk in African Americans with ruptured abdominal aortic aneurysms. Females were also less likely to undergo endovascular procedures.

In 2022, only 195 of the ~800 participating facilities had documented use with the vast majority at level 1 centers, a limited number at level 2 centers, and virtually non-existent at level 3 centers.
