#AI rounds in #Pedineurorad: This week we review another mitochondrial disorder driven by ECHS1 (Enoyl-CoA Hydratase Short Chain 1) deficiency. ECHS1 is a key enzyme in mitochondrial fatty acid beta-oxidation, specifically catalyzing the second step: the hydration of trans-2-enoyl-CoA to 3-hydroxyacyl-CoA. This reaction is necessary for breaking down short- and medium-chain fatty acids into acetyl-CoA, which then enters the tricarboxylic acid (TCA) cycle to produce ATP. Diagnosis ECHS1 deficiency is diagnosed through a combination of: 1.Clinical Presentation •Progressive neurodevelopmental regression •Hypotonia, dystonia, spasticity •Seizures and encephalopathy •Lactic acidosis 2.Laboratory Findings •Metabolic Screening: •Elevated lactate in blood and cerebrospinal fluid •Abnormal acylcarnitine profile, particularly accumulation of C4-OH and C5-OH acylcarnitines •Increased urinary excretion of branched-chain ketoacid metabolites •Enzyme Assays: •Measurement of ECHS1 enzyme activity in fibroblasts or muscle biopsy •Deficiency of short-chain enoyl-CoA hydratase activity confirms the diagnosis 3.Genetic Testing •Whole-exome sequencing (WES) or targeted gene panel sequencing to identify pathogenic variants in the ECHS1 gene •Autosomal recessive inheritance: Both copies of the gene must be mutated 4.Neuroimaging (MRI Brain) •Bilateral basal ganglia lesions, particularly affecting the putamen •Leigh syndrome-like patterns of neurodegeneration Pathophysiology: Why Does ECHS1 Deficiency Cause Disease? ECHS1 is crucial for two major metabolic pathways in the mitochondria: 1.Fatty Acid Beta-Oxidation (FAO) •Converts short- and medium-chain enoyl-CoA derivatives into hydroxyacyl-CoA •Deficiency leads to impaired energy production, especially in high-energy tissues like the brain and muscles 2.Branched-Chain Amino Acid (BCAA) Metabolism •Plays a role in degrading valine, leucine, and isoleucine •Disruption leads to accumulation of toxic intermediates, causing mitochondrial stress and neurotoxicity Key Consequences: •Accumulation of toxic metabolites (e.g., S-(2-carboxypropyl)-cysteamine) •Energy deficiency, particularly in high-demand tissues (brain, muscles) •Mitochondrial dysfunction → oxidative stress, neuronal damage, and progressive neurodegeneration. Treatment & Management Currently, there is no cure for ECHS1 deficiency, but supportive treatments aim to manage symptoms and improve quality of life. 1.Dietary & Metabolic Management •Low-fat diet: Reducing reliance on fatty acid oxidation •Branched-chain amino acid (BCAA) restriction: Lowering intake of leucine, isoleucine, and valine to reduce toxic metabolite accumulation •Supplementation: •Ketogenic diet (used in some cases, but controversial due to FAO impairment) •Cofactors & antioxidants (riboflavin, CoQ10, lipoic acid) to support mitochondrial function 2.Symptomatic Treatments •Seizure management with anti-epileptic drugs •Muscle tone control (baclofen, botulinum toxin for spasticity/dystonia) •Physical therapy & supportive care 3.Experimental & Emerging Therapies •Mitochondrial-targeted therapies: •NAD+ precursors (e.g., nicotinamide riboside) •Mitochondrial protective compounds (CoQ10, idebenone) •Gene therapy approaches (future research focus) Prognosis •Highly variable, depending on mutation severity •Severe cases present in infancy with rapid neurodegeneration and early death •Milder cases may survive into childhood with supportive care
