Writing up the simple version of our interop and got a bit nostalgic. FHIR has replaced where I would have done MUMPS functions. The top part is basically a GP system from the 90's :) The bottom part is acute trust from the 90's.

Love it how as a developer, I distracted by shinny things. Just double checking a few bits of code and noticed how close we are to being ready for Apple Care Kit and Apple Health Records. Was only doing a one job and been looking at videos and docs for hours.

Oddly a fix to a problem I tried to report has appeared, tried several different channels but got nowhere. When I worked for a GP supplier, this would have had a very very short turn around. It might have been me pointing out the issue which triggered the fix, I don't know.

Not saying on boarding takes long but celebrating a go live!!! Wonder if we are the first HL7 IPA based in the UK? Code is a couple of months shy of becoming a one year old.

It's not 100% HL7 IPA query API, it is a facade to another system and so has some limitations.

45 mins later, o2 have swapped my contract to SIM. Why can't do this via internet in 1 minute????

COVID recovery taking a little longer. Experimented with cardio exercise on Friday, to generate some 'load' on my body. Resting heart rate going down :)

Having a bit of break from X/Twitter. First post I read has racial hatred overtones. I think I need to stop coming back to this swastika.com

I suppose if we go back to 878AD. HS2 has a pretty awesome coverage of the whole of Southern England. Danes held them again at the Trent. Welsh, Scots/Picts, Danes, Saxons and Northumbrians (Northern Angleland) have didly squat though?

I get a bit complicated with this but we are generally have low level interop issues with three big EPR suppliers and also apple in the UK. Many others in secondary care and also regional services may also be good enough for us to work with at a low cost.
Think I'm altering my definition of technical interoperability basic principles in England to be following information standards such as: UK SNOMED CT and NHS Data Dictionary Most technical standards which use these such as HL7 v2 ADT, DICOM and XDS (IHE UK) are ok.

It's following the basic principles which makes this easier for us. Not FHIR, v2, DICOM or XDS - but using just one of those also reduces our costs.

FHIR shouldn't really have medium/major transform issues with HL7 v2, XDS or DICOM. Ideally "It's just the JSON version" Most of the issues are around low level data types which can the same between the standards but have been modelled differently.

These low level data types (CX, CN, CNE, CWE, CQ or FHIR Identifier, CodeableConcept or Quantity) have to follow same information standards (they are designed to be technically compatible), i.e. NHS Data Dictionary, ICD 9/10, SNOMED, LOINC, UCUM, etc.

Doing some coding 'therapy' for a community provider. Do miss working in this area.

Wondering about making some big carer decisions. Had a bit of moral failure when I realised how many of the problems I’m looking at are quite basic when you removed the trimmings, learned them very early on in my carer 30 years ago. Could do with something more cutting edge.

This is looking quite complex. Main thing I've done is split what is already around into: - Business view - Technical overview Wanting to shift to business view due to 'big ball of mud' issues around tech.

Computer system names and also tech standards are also being described around use cases and actors. Still a work in progress

Quite like how Flanders approaches pedestrians, car and cyclists: Do people use the footpath, yes - don’t park on footpath. Do most people walk or cycle on street, yes - don’t allow on street parking (for safety reasons) and maybe, limit/ban cars.

Is cycling on road dangerous or is cycling on pavement dangerous to pedestrians, yes - build segregated cycle path (Or recommend cyclists to not use) Is cycle path to get people to shops or schools or for leisure/exercise which non enthusiasts will use - No don’t build.

Must be some car rules because driving and parking was a lot quicker and easier. I think it was around making some roads/paths suitable for pedestrians/cyclists and on others just focused on cars. Didn’t try to keep everyone ‘happy’ on every road?
