Fewer than 7% of physicians feel prepared to manage menopausal women.
Not for lack of caring. No one taught us.
My Hormone Optimization Therapy (HOT) Provider virtual course is now accredited for 15 CME/CE credit hours. 🎉

Dermatology is wrong about the sun.
And it's killing people.
I'm a dermatologist. 226 publications. I should know.
Avoiding the sun increases the risk of dying as much as being a smoker.
We can fix it.
For decades, dermatology's message has been simple: avoid the sun. Wear sunscreen. Seek shade. UV causes skin cancer. End of discussion.
That message is incomplete and outdated.
People are dying because of it. Lots of people.
The evidence has gotten strong enough that the field needs to update it.🧵

1/ Amy Killen, MD’s sharpest lens: women’s aging doesn’t show up all at once. Some of the earliest shifts happen upstream—then echo outward into energy, recovery, body composition, cognition, skin, bone, and sexual health.
One time discount at the end.
Thank you @AnthropicAI and @DarioAmodei for not bending to our insane administration and agreeing to use your AI in autonomous weapons and mass surveillance.
As a physician at the intersection of technology and health, I care about privacy.
As a human, I care about not letting AI pull a trigger when it still fabricates PubMed citations with a straight face.
@OpenAI - I wish I could do more than just cancel my ChatGpt. You should be ashamed.
Are you worried that quality, body identical hormones are too expensive?
What if I told you that you can potentially get all of your menopause hormones for less $20/month without insurance? 😍
Yeah, I'm talking low dose vaginal estradiol, systemic estradiol, progesterone and testosterone for less than $20.
What?!
Check out this week's Free Substack with the details. 👇

Is FSH a useless lab or missing piece in menopause hormone users?
A recent Korean study looked at a large group of postmenopausal women (not on hormones) who'd had amyloid PET scans. They found higher serum FSH levels were associated with worse cognitive function and more amyloid plaque (Wang et al, Front Aging Neurosci, 2026).
This was observational but adds to a growing body of literature that points to FSH as not only acting as an indicator of end organ estrogen exposure but having its own negative effects on tissues like the bones, adipose and brain.
This week's Substack explores this topic.

Wait. You're telling me that the Olympic ski jumpers who injected hyaluronic acid into their 🍆's did so to make them fly further?
Not to look bigger in their skintight suits but to go further in the air?
Do we think that the pole vaulter from 2024 was thinking the same thing but over-shot?
Is there a J-shaped curve I should know about that describes the perfect amount of bulge needed to fly but not catch?
Why would Attia's "fresh shipment" be in regards to metformin?
Metformin is a readily available, generic, $9 drug.
I'm confused.
Does where you apply testosterone cream/gel influence side effects?!
Yes!
Skin with high numbers of sebaceous and sweat glands also tend to have higher 5a reductase activity, which means more testosterone gets made into DHT.
While DHT is responsible for many of the much-needed androgenic benefits in men, it's often associated with side effects in women, including acne, body and facial hair growth and scalp hair loss.
Avoid applying testosterone cream/gel to areas such as the face, chest, axilla, groin/external genitalia in women. Opt for inner arms, lower abdomen, inner/outer thighs.

I'm supposed to talk about hormones, sexual health, and longevity.
These are the things you want to hear from me. Quick tips and cool tricks. So, I will.
Do you know what's bad for your hormones, your sex life, and your longevity?
Getting shot on the street by cowardly men in face masks.
We can't pretend to care about health and wellness but continue to ignore innocent people getting executed.
If you have a BRCA gene variant you've probably been told you can't take hormone therapy. Your doctor may have said it's "too risky". That estrogen will "feed" a future cancer. That you just have to white-knuckle your way through surgical menopause at 40.
This is wrong.
It's been wrong for years and a recent study presented at the San Antonio Breast Cancer Symposium confirmed what the days have been quietly showing for two decades: hormone therapy does not increase breast cancer risk in this population. Estrogen may actually reduce it.

In 2011 I saw an ad in an in-flight magazine that changed everything! ✈️
The ad by @cenegenics featured a muscled-up physician named Dr Jeffrey Life and was titled "How does this 73 year old doctor have the body of a 30-year old?" 💪
Dr Life was promoting a new kind of medicine called age-management medicine that used exercise, nutrition and hormone optimization to improve health and prevent the diseases of aging. 🤯
This ad changed my life.
I wrote about it (and one of my first HRT patients) in this week's FREE Substack.

A few weeks ago I attended the Global Longevity Summit and heard an incredible talk by Louise Hecker about her recently published study on psilocybin's longevity benefits in mice (Hecker, L et al, NPJ Aging, July 2024).
We're talking about psychedelic monthly doses of psilocybin extending lifespan significantly AND improving skin and hair.
She also commented on micro-dosed psilocybin (weekly) and its impressive skin and hair benefits.
I've broken it all down (and added a bit of local flavor) in this week's Substack

‼️ Once you hear this, you can't go back.
😓 You think a night sweats are “just part of being a woman”
Think again.
🧠 Your blood vessels are in the corner doing the Harlem Shake and leaving little micro-strokes as souvenirs.
What's most alarming?
👀 The medical community has known this for years.
🤔 So why are they still letting women suffer?

P.S. Correction - it's "white matter hyperintensities" not hyperdensities. Apologies for the confusion. ✅
.
.
What if I told you that hot flashes and night sweats are not just uncomfortable.
What if I told you these vasomotor symptoms were strongly associated with acute increased white matter hyperintensities in the brain?
And, those white matter defects are highly associated with stroke and dementia.
Oh, and that we've known this for years but, for some reason, we keep letting women have hot flashes and night sweats (even though we have an excellent, safe drug that is highly effective in stopping them).
