Quick takes on the Social Signals from #ASCO26. 🧬 RASolute-302: @marklewismd 's tweet regarding the standing ovation for RASolute-302 really made that trial stand out. The momentum here was massive, fueled by high expectations from the @RevMedicines PR, #AACR26 presentations, and the high-profile Ben Sasse interviews. ⚔️ PROTEUS: One of the more interesting trials from a social media perspective, as the radiation oncology community on X was highly critical of the trial's surgical paradigm. 🔄 CROWN: Felt a bit like an incredible replay of the Plenary from years ago—steady, practice-confirming data. 🎬 HARMONi-6: Perhaps the most compelling narrative twist. The discussant, Dr. Julie Brahmer, masterfully set up the Plenary audience using The Devil Wears Prada as a research metaphor. It begs the question: Is VEGF back? 🚀 LIBRETTO-432: A massive surprise for many attendees. Lung cancer specialists were thrilled about @LillyOncMed selpercatinib in the adjuvant setting for RET-positive NSCLC. The core takeaway from @christine_lovly presentation was loud and clear: "This is why we test." 🔬 OPTIMA: Really got the breast cancer community excited about using @Veracyte 's test to safely reduce the chemotherapy burden for many of their patients. 📦 WU-KONG28: Had such an interesting vibe. Great data from @Dizal_Global , but it leaves the KOLs asking the same commercial question: Why isn't this readily available to patients in the US yet? NADINA HR 0.4 SWOG 1801 HR.58 - few doses of neoadj IO while primary is intact appears to be way better than jumping through the hoops to make individualized neoantigen vaccines on top of adj IO.

🚨 ESMO 2026 | TALENT Trial (LBA23) Disappointing results for neoadjuvant T-DXd in HR+/HER2-low early breast cancer ? 🔹 T-DXd alone: pCR/near-pCR 16% 🔹 T-DXd + anastrozole: 17% 🔹 T-DXd → TC chemotherapy: 17% Despite ORR ~65–67%, pCR rates remained low. Adding endocrine therapy or sequential chemotherapy did not meaningfully improve response. Looking forward towards full dataset. @OncoAlert @myESMO @OncBrothers #ESMO26

HR 0.58 is right where the phase 2b was, and they used a real pembro control, which I give them credit for. Unfortunately this is RFS on top of a pembro backbone that was itself approved on RFS. COMBI-AD showed big adjuvant RFS and DMFS gains with no significant OS difference. Patients who recur now have good salvage options, so treating earlier isn't necessarily curing more people. Fever in 52% vs 8% means nobody was really blinded. I'd want OS before calling this a new standard.
These results with adjuvant Moderna personalized vaccine added to pembro for melanoma look good to me, and are a positive indicator for the adjuvant and perioperative lung cancer trials. #LCSM

I feel like news coverage that used to serve a more well-rounded audience, including clinicians, patients, and skeptics, has become increasingly focused on investors. Maybe it’s just my perception, but I sense a shift.

Who is actually served by a topline trial announcement that essentially says only “endpoints met”? Investors? Would Moderna’s August announcement have included more detail if the Phase 3 DMFS results had been closer to Phase 2b?

✈️ En route to Madrid for #ESMO26, landing just in time for the data to start Friday morning. Most excited for: VOLGA LBA2 in bladder cancer, INTerpath-001's first phase 3 neoantigen win, LITESPARK-011 final in kidney cancer, and DASL-HiCaP in prostate. What a week ahead 🔬 #GUonc

‼️ Interesting commentary on INTerpath-001 No need for vaccine if pembro given neoadjuvant? Wow, await further details / data here, but what a dud! Precision/Personalized dud
$MRNA with the single most hilarious possible outcome in this study. EFS HR 0.58. Down the the hundredth, EXACTLY what *NEO*-adjuvant treatment with pembro yielded against adjuvant pembro (the nerfed comparator $MRNA studied against) in SWOG S1801. What does this mean in English? $MRNA’s drug is showing the exact same benefit versus adjuvant pembrolizumab as NEOadjuvant pembrolizumab did. Even more simply? If you just give patients a few doses of the pembrolizumab BEFORE you cut their cancer out, you do what $MRNA does for you, except with just the free tumor that’s already inside your body. No need to pay for this $400K drug. And this makes rational sense, right? The whole point of $MRNA’s vaccine is to produce stuff that looks like the tumor. You know what looks A LOT like the tumor does? THE FREAKING TUMOR!!!!!! Absolutely poetic that $MRNA produced the EXACT HR benefit as neoadjuvant pembro did. Truly, the most hilarious possible outcome. Now can we move on from this bullshit? $MRK and $MRNA should feel free to study their vaccine against the REAL standard of care now - NEOadjuvant immunotherapy. From my perspective, this has been total fugaze, and an amateurish pump and dump worthy of much lesser enterprises.

🚨 ESMO 2026 | INTErpath-001 (LBA1) - as data is released by Esmo 🔹 24-month RFS: 82.2% vs 70.0% HR 0.58 (95% CI 0.45–0.75); P<0.001 🔹 24-month DMFS: 88.5% vs 80.2% HR 0.62 (95% CI 0.45–0.85); P=0.002 🔹 Grade ≥3 adverse events: 23.6% vs 20.3% The trial randomized 1,137 patients with resected stage IIB–IV melanoma. Both arms received pembrolizumab; the experimental arm added an individualized mRNA neoantigen therapy. The key question now: will better recurrence control translate into an overall survival benefit? Looking forward to full dataset on day 2 . #ESMO26 @myESMO

🚨 What a great start to #ESMO2026! 🇪🇺 The European Commission approves perioperative enfortumab vedotin + pembrolizumab for resectable MIBC, regardless of cisplatin eligibility! A major milestone for our patients. Now looking forward to VOLGA at the Presidential Symposium on Saturday! @tompowles1 With only 3 neoadjuvant EV cycles and no adjuvant EV, VOLGA could provide important insights into a key question: How much EV do we really need? Does adjuvant EV add meaningful efficacy—or mainly cumulative toxicity? Cross-trial comparisons cannot answer this definitively. Biomarker-guided de-escalation trials are urgently needed! 🔬 @DrChoueiri @Markuseckstein3 @DrRosenbergMSK @Dr_Aggen @tompowles1 @urotoday @UroDocAsh @Uromigos @weoncologists @OncoAlert @raffcolo @PTarantinoMD @drenriquegrande @DrYukselUrun @AndreaNecchi @apolo_andrea
