Ok on this note…what are the best examination features of severe AS? I’ll go first… I was once taught my someone ‘old school’ to feel for apical-carotid delay. I’d recommend it. If you can feel maximal apical pulsation BEFORE the carotid it’s always severe AS. Simples

And shout out to those whose help was invaluable when writing it… @shaunrobinson02 @DanXAugustine @dr_benoy_n_shah @Echo_Imaging @oxborough77
RE the AS discussion (and to toot my flute)…this took A LOT of time to write…it was shredded several times by reviewers…but after much blood sweat and help from coauthors I’m very proud of it. Even though only about 10 ppl have read it 🙄😂 echo.biomedcentral.com/artic…

Ummm….none of these 😂😂 Vmax or mean gradient.
If one would choose the best AS severity measure, you choose:
20%1. AVA
25%2. AVA/BSA
29%3. DVI
26%4. Vmean

Completely agree. Underrated. Underused. Physiologically makes sense. Certainly the best data (in my opinion) for interpreting AS in what is a phenomenally challenging group of patients.

Ummm…this is beyond disturbing. Government tried to cancel education conferences because lead speakers disagreed with government policy. Also keeping ‘files’ on individuals who speak out against DfE and government: The Guardian apple.news/A-KHd5YD7QfGaXdmB…

At least these days there’s no more ‘white coat hypertension’ 🙄😬😂
Could never really understand (or believe the reasons given) that white coats had to to go!

Love this…didn’t we use to call those ‘echo beats’…? P waves not regular because the retrograde ‘P’ resets the sinus node. Also the P wave buried in the terminal aspect of the third QRS doesn’t have the same morphology as the others: it’s negative in lead II (a pseudo ‘S’)!
#ECG OK, arrhythmia nerds - what exactly do you think is happening here? I'm not certain, but I do have a possible explanation.

A little something that everyone wants to see on a Sunday: who can identify three diagnoses in this video? Also (shameless act of promotion alert)….if you get the answer AND come to @BSEcho conference I will personally buy you a beer (other drinks are available🙄)

1. Anterior MI (ST elevation on the ECG - not easy to see). 2. Complete heart block. 3. Apical rupture with haemopericardium. No LVH here - this is ‘pseudohypertrophy’ because the LV is so empty - not a surprise given the massive hole! Now…who’s for that beer at @BSEcho?

Big shout out to the @BSEcho team for the inaugural registrar training day. A huge amount of time and effort has gone into this…but well worth it.
We've had a great day today in Newcastle! Thanks to all our faculty for their hard work and expertise

Thanks @vass_vassiliou for the shout out! Yes…I’ve decided that Twitter is finally ready for me 😬. I’m looking forward to connecting with the online cardiac and echo community….and hopefully not embarrassing myself too much in the process. @BSEcho
