Neuroscientists have identified the specific spinal cord neurons that enable people with paralysis to walk again. In a Nature study from the NeuroRestore center, researchers found that a specialized group of interneurons expressing the Vsx2 gene is essential for restoring motor function after chronic spinal cord injury. Nine participants with long-term injuries underwent targeted epidural electrical stimulation combined with intensive rehabilitation. All nine regained or improved their ability to walk, and the gains persisted even after stimulation was switched off, indicating that spinal circuits had reorganized. Mapping the spinal cord in animal models showed that these Vsx2-expressing neurons are not required for normal walking in healthy animals but become critical for rebuilding motor pathways after injury. Activating them promoted recovery, while silencing them prevented it. The findings point toward more precise molecular therapies, implants, and rehabilitation approaches for people living with paralysis. [Kathe, C., Skinnider, M. A., Hutson, T. H., et al. (2022). The neurons that restore walking after paralysis. Nature, 611, 540–547. DOI: 10.1038/s41586-022-05385-7]

In the Spring issue of the NeuRA Magazine, we speak to Sydney Brain Bank Director, Associate Professor Claire Shepherd, on how the brain donations are helping tackle our brain health challenges. Also, NeuRA's Professor George Paxinos and Dr Steve Kassem propose a 'periodic table' for brain structures, how holistic pain treatments tackle links between pain and wellbeing with Professor Sylvia Gustin, we remember Marc Girod, and Ask a Researcher with NeuRA CEO Scientia Professor Matthew Kiernan AM. Based on the article "ALS Pathogenesis and Therapeutic Approaches: The Role of Mesenchymal Stem Cells and Extracellular Vesicles" (Bonafede & Mariotti, 2017): In ALS, glutamate excitotoxicity occurs when excessive extracellular glutamate hyperactivates postsynaptic receptors, driving a massive intracellular influx of calcium (Ca2+) into motor neurons. This toxicity is exacerbated by the loss or downregulation of astroglial glutamate transporters (EAAT2/GLT-1), which prevents efficient glutamate clearance from the synaptic cleft. Simultaneously, motor neurons suffer from impaired mitochondrial calcium homeostasis—compounded by mutant SOD1 and a loss of calcium-binding proteins—reducing their capacity to buffer cytosolic calcium. This uncontrolled postsynaptic calcium overload triggers structural mitochondrial damage, elevated ROS production, and enzymatic cascades leading to motor neuron apoptosis.
