Hi everyone, I’m Marvellous, some of you know me as Ini. I’m a few months from finishing my MPH at NYU. I planned for this year with a student loan and a summer internship but it didn’t come through. Then this month, MPOWER shut down. I was on their waitlist. #FundDrMarvellous

Bioequivalence in a country like Nigeria with fake drugs epidemic and poor regulatory systems. Let’s be honest with ourselves.

An important confounder however is that a patient who can afford 12 hourly dosing of Rocephin, would most likely present earlier, can afford important investigations and treatment modalities, thereby improving clinical outcomes.

It's funny how we are always trying to find a “gotcha moment” against one another among Nigerian healthcare workers. Our patients aren't better for it. The context of the doctor's tweet is the prevalence of fake and substandard products, as well as concerns about the quality of locally manufactured products. Admittedly, there will be exceptions, but these concerns cannot simply be dismissed. His statement isn't necessarily borne out of ignorance or malice. It reflects anecdotal evidence and clinical experiences of patients responding differently to different brands of the same drugs. This is an experience shared by doctors, pharmacists and patients alike. With that being said, the response to this kind of conversation should be education, not belittlement; intellectual, not sentimental; and respectful, not condescending. Professional disagreements should be opportunities to exchange knowledge and improve practice, not occasions to demonstrate superiority over colleagues. The goal should be better patient outcomes, not winning arguments against one another.
Our training is so different The knowledge gap is crazy

I have been avoiding this conversation since yesterday but it seems you guys are happy pretending we don't have a standardisation problem across Nigerian sectors including the drug industry. So here's global peer reviewed evidence that generics are not universally bioequivalent. Even the local research in Ibadan had "a few cases" Finally let's remember that because it happened in the lab doesn't mean it will happen in-vivo.
You say this everytime. Kindly furnish me with your clinical data that warrant such lies.

I hope the furosemide 'fest' brand whose company is in kano and is not in NAFDAC greenbook has same bioequivalence with lasix? I don't even have these details, this is why I clamour for very good pharmacist grounded in pharmaceutical evidence and I want to learn from them.

Replying to @Walk_yea
For sometime I struggled whenever my Pharmacy colleague taught a particular module on our Global Health course and will say similar things to what the Doctor said. This year he came with enough evidence to support his stance and I had no words. The truth is, in many emerging markets, there a lot of substandard generics in circulation. Denying that is pretending that we do not see what happens daily in our hospital wings. We can't put our heads under the sand and pretend that we have organised standards yet we have a massive fake drug problem.

Guess I’ll spend a part of my weekend digging on Rocephin vs other brands of Ceftriaxone.
There's a reason why many consultants usually prescribe brand name of drugs. 1g of rocephin is not 1g cefriazone. 625mg of Augmentin is not 625mg of amoxicillin+ clavulanic 5mg Norvasc is not 5mg amlodipine. 80mg Arbitel is not 80mg Telmisartan IYKYK

Had this same argument with some colleagues a few days ago. “Residency-shaming” people is not it at all. Not everyone will be and want to be specialists and that’s not mediocre.
We need to stop making it seem like anyone who doesn’t go for residency is making a bad decision. Let people do their thing please.

MDCN is now restricting the issuance of Certificate of Good Standing to 5 years post graduation from medical school. In other words, they won't verify your licence to practise to register or practise as a doctor in another country until 5 years after graduating from medical sch
