🧬 OrigAMI-4 at ESMO 2026: 77% Response Rate With Amivantamab, Pembrolizumab, and Carboplatin in Head and Neck Cancer
Could combining EGFR-MET bispecific targeting with immunotherapy and chemotherapy improve first-line treatment outcomes in recurrent or metastatic head and neck cancer?
At ESMO Congress 2026, Paul Swiecicki presented results from the phase Ib/II OrigAMI-4 trial, evaluating amivantamab plus pembrolizumab and carboplatin in recurrent or metastatic head and neck squamous cell carcinoma (HNSCC).
🔬 Key Findings:
🔹 HPV-unrelated disease: Confirmed ORR of 77% (17/22), including three complete responses.
🔹 Response durability: 65% of confirmed responses in HPV-unrelated disease remained ongoing at the data cutoff.
🔹 HPV-positive oropharyngeal cancer: ORR of 29% (2/7).
🔹 Safety: The most common adverse events included anemia, hypoalbuminemia, thrombocytopenia, stomatitis, and hypomagnesemia.
🔹 Administration-related reactions: Reported in 13% of patients, with no grade ≥3 events.
Clinical Takeaway: OrigAMI-4 demonstrated encouraging preliminary activity, particularly in HPV-unrelated HNSCC. While the small cohort and short follow-up limit conclusions, these findings have supported the phase III OrigAMI-5 trial comparing this combination with standard first-line treatment.

Phase II REPRINT: Enfortumab Vedotin Plus Pembrolizumab in Rare Renal Carcinomas
At the ESMO Congress 2026, Giuseppe Procopio presented preliminary first-stage results from the Phase II REPRINT trial (NCT06302569), evaluating enfortumab vedotin plus pembrolizumab in patients with collecting duct carcinoma (CDC) and renal medullary carcinoma (RMC).
Key findings:
▫️ Study population: 13 patients enrolled; nine evaluable after central pathological review (seven CDC and two RCCU-MP).
▫️ Objective response rate: 22% (2/9), with both responses occurring in patients with SMARCB1-deficient RCCU-MP. No objective responses were observed in CDC (0/7).
▫️ Disease control rate: 55% (5/9), including two partial responses and three cases of stable disease.
▫️ Median PFS: 5.2 months (95% CI 3.4–not reached).
▫️ Safety: No Grade 3–4 adverse events were reported.
▫️ Biomarkers: Both responding tumours were negative for membranous NECTIN-4 expression.
Takeaway: REPRINT demonstrated preliminary antitumour activity and met the prespecified criteria to proceed beyond the first stage of Simon’s two-stage design. The findings are encouraging in the small RCCU-MP subgroup, but further evaluation is needed to establish efficacy and safety.

🧬 SMART-KEY at ESMO 2026: 65.1% Laryngectomy-Free Survival With Pembrolizumab-Based Treatment in Advanced Laryngeal Cancer
Can immunotherapy help preserve the larynx while reducing the need for cisplatin-based chemoradiotherapy?
At ESMO Congress 2026, Thiago B. Oliveira presented results from the phase II SMART-KEY (LACOG 0720) trial, evaluating a pembrolizumab-based, cisplatin-free organ preservation strategy in patients with locally advanced laryngeal cancer.
🔬 Key Findings:
🔹 2-year laryngectomy-free survival: 65.1%.
🔹 Larynx preservation: 86% at two years.
🔹 Laryngeal dysfunction-free survival: 83.7%.
🔹 Overall survival: 74.4% at two years.
🔹 Disease recurrence: Locoregional recurrence occurred in 18.8% and distant recurrence in 7.4% at two years.
🔹 Safety: Grade ≥3 treatment-related adverse events occurred in 41.9% of patients, including one treatment-related death.
Clinical Takeaway: SMART-KEY demonstrated encouraging organ preservation and survival outcomes without concurrent cisplatin-based chemotherapy.
However, its single-arm design and historical comparisons mean further randomized studies are needed to establish whether this approach can safely replace standard chemoradiotherapy.

Bladder Cancer Grand Rounds happening in Madrid! 🇪🇸
Two sessions bringing the latest evidence and worldwide clinical experience together at OncoDaily Grand Rounds during ESMO 2026.
Part 1, chaired by Prof. @AndreaNecchi, featured educational lectures on:
• Radical cystectomy and bladder preservation — Dr. @DrFelixGuerrero
• Systemic therapy advances in localised bladder cancer — Prof. @AndreaNecchi
• Treatment selection and sequencing in advanced urothelial carcinoma — Dr. Sara Coca Membribes (@scocmem)
• Practical management, toxicity and monitoring — Dr. María José Juan Fita
Part 2, chaired by Dr. María José Juan Fita, brought worldwide experience to the discussion through shared clinical cases. The panel explored the use of enfortumab vedotin plus pembrolizumab, perioperative strategies, treatment sequencing and bladder preservation and how access and reimbursement shape everyday decisions.
Joining the discussion were Dr. Sara Coca Membribes (@scocmem) (United Kingdom), Dr. @DrFelixGuerrero (Spain), Dr. Helena Kremer (Brazil), Dr. Kelvin Yan (Hong Kong, China), Dr. @DrUddiGoswami (India), Dr. Samvel Bardakhchyan (Armenia), Dr. Lazar Popovic (@LazarPo31787807) (Serbia), Prof. Chunguang Yang (China), Dr. Javier Molina Cerrillo (@JaviMolinaC) (Spain) and Dr. Kleida Mati (Albania).
The same clinical scenario can lead to very different treatment choices across countries. Sharing these experiences helps us understand the gaps and what we can do better together.
Thank you to all our faculty and participants for such an open and engaging discussion!
This independent educational activity is supported by a grant from @AstellasUS.
