Iβm at UCLA
Why did they drop this?
Here is my guess
Medical patient (LA Health) comes to ED with fever right lower quadrant pain and tenderness. CT shows appendicitis. Path on appendectomy specimen shows appendicitis. Admission denied: not medically necessary. Patient calls me in extreme distress after being told this by his insurance company
In other postings I related the story of responding to an ED consult in the middle of the night, doing a full H&P, removing a foreign body and providing follow up. Reimbursement $40.
Medical (Californias version of Medicaid) is unsustainable given that it reimburses for less than the cost of health care delivery.
That the solution to the uninsured was to expand Medicaid in the ACA was a joke to any clinician who deals with these realities.

A faculty member cut her research effort in half over three years. No one told her to. The comp plan did it for her. RVU-only compensation has destroyed more research capacity at academic medical centers than any funding cut.

It's definitely worth a read if you perform open and/or MIS proctectomies for #RectalCancer

Behind every surgeon is a collection of people who helped along the way.
The ASCRS General Surgery Mentorship Program is an opportunity to pay that forward and help ensure the next generation experiences the same support.
Whether you're a colorectal surgeon looking to share your experience or a general surgery resident exploring the specialty, there's still time to get involved.
Applications close next Wednesday, July 1.

Another very important question is that are fellowships training too many specialists. Are they getting those select specialized jobs at the end of training. What we are seeing in HPB is that many graduates wind up at community practices with low volume of those cases.
This post from @lee_c_zhao is outstanding, and IMHO a critical assessment in any surgical traineeβs journey (also, read the entire π§΅).
Thanks Lee. ππΎ #MedEd

Amen Sy. Pediatric Surgery was historically ahead of the curve when they tied the number of training positions each year to the job market.
Itβs a huge disservice when we train too many specialists. Dilutes quality, and leads to frustration on so many levels.

One conversation can change the course of a career.
For a resident, it can provide clarity, confidence, and a glimpse into the future. For a surgeon, it can be an opportunity to share lessons learned and help guide the next generation.
The ASCRS General Surgery Mentorship Program connects colorectal surgeons and general surgery residents through meaningful mentorship that strengthens both individuals and our specialty.
Applications for mentors and mentees are open.
