This was another case that I came across a while ago, which shows pleomorphic CIN3. There was no invasion in multiple levels examined.

Pleomorphic CIN. This is just a LG CIN showing striking pleomorphism of basal cells. Limited studies, but not associated with aggressive behaviour and may even represent a degenerative change. Can be confused for early stromal invasion when cross cut. #pathtwitter #gynpath

Prophylactic hysterectomy for LS. All benign. But saw this in one of the sections. This couldn't be fat. Could it? Have you ever come across this? #gynpath @Dramanyhassany1 @slusagar @kis_lorand @GbGynae @OBGYNPATH @DrSwatiBhardwaj @DrEstherAdler @DrGeeONE @TristanRutland7

F/50ish vaginal polyp. p16 negative. GATA3 positive in squamous, PSA negative. Do we have a name for this? Would you call it a tubulo-squamous polyp, or does the dominant glandular component speak against it? #pathtwitter #Gynpath

Most impressive p53 that I have ever seen! (first image) And a negative p16! (second image) So a great example of HPV-independent VIN mimicking HPV-associated VIN. There is a hint of stromal hyalinisation in the first HE image and some lichenoid inflammation as soft hints!

F/64 ish Vulval biopsy These HE images are enough to make the diagnosis! HPV-A HSIL/VIN 3. Right? Check the next tweet/post in the thread. It may surprise you! #pathtwitter #gynpath #frcpath #histopathology

2/3 The first image is PAX8 and the second one CK7. ER negative. So reduced markers of differentiation in the undiff component. MMR intact. Neuroendocrine markers negative but wouldn't have surprised me if there was focal positivity as @kis_lorand says.

Focally, the solid spindle cell morphology was also noted. And as many of you rightly suggested the diagnosis of ENDOMETRIOID CARCINOMA RESEMBLING SEXCORD STROMAL TUMOUR was reached. A well-described albeit rare morphological variant and a diagnostic pitfall. 2/3
