He walked in saying his back hurt after lifting something heavy. Classic story. The ER doc did the usual exam, gave him some pain meds, and was about to send him home with “muscle strain.” Guy was in his late 30s, no real red flags at first glance. He kept mentioning his legs felt a little weird, like they were getting heavy, but nobody pushed it.
Someone finally ordered an MRI of the spine.
The scan showed a big disc herniation crushing the cauda equina. He was starting to lose function in his legs and bladder. They got him into surgery that night. He walked out a few days later with most of his strength back. If they’d waited for the classic “saddle anesthesia and incontinence” picture, it might’ve been permanent.
A lot of back pain is just back pain, and most people don’t need an MRI right away. But when someone’s young and the weakness is creeping in even if the usual warning signs aren’t fully there yet, the MRI is the one that actually shows the cord and the nerves getting crushed. Plain films and clinical exams miss this stuff all the time until it’s too late.
Not every sore back needs a scan. But when the patient says their legs feel wrong and the story doesn’t quite add up, that MRI is often what stops “take some ibuprofen and rest” from turning into permanent damage.

A 63-year-old man who had an esophagectomy for cancer years ago presents with mild dysphagia. Chest x-ray showed an unusual mediastinal contour, so contrast CT of the chest was done.
What is the diagnosis?
