Our job as experts is to unpack intuition: What are we seeing? What patterns are we recognising? What makes us change course?
The ultimate demonstration of mastery is not knowing what to do, itβs being able to explain why, and teach someone else to do it.

Calling expertise a βsixth senseβ or βmuscle memoryβ risks turning mastery into mystique.
Yes, complex airway management demands experience. But expertise that cannot be articulated cannot be reliably taught, examinedβor improved.
Another fantastic annual meeting under our belt! Special thanks to overseas guests @altgm @AndyHiggsGAA & @sthjournalclub as well as to @DrRaniChahal for coordinating our 1st partnership w the @ANZCA @theNZSA @ASA_Australia Airway SIG.
SAS will be back in Noosa for #SASNoosa27

So, the question becomes βWhy not apply it?β
I wrote a (slightly facetious) letter suggesting we should be using CP routinely, irrespective of aspiration risk.
The only reason is really lack of staff (as Iβd advocate it should be a dedicated role).

Interesting
Not what I do but interesting
I think there may be subtleties between different HAVL blades that makes this technique more suitable for some than others
There are certainly differences between blades!

If I might also offer there is something that could be usefully added
-head and neck position
I believe a key reason folk go on about difficult intubation with good view and difficult tube delivery is failure to position H&N correctly
Flextension

Exactly why the @UniversalAirway guidelines ask you to instead make an individualised assessment of whether management will be βrapid & reliableβ. The words βdifficult airwayβ do not appear in any of the guidelines.

This line in the Foundation guideline (citing the same paper) paraphrases the comment you made to me that inspired me to lift my HAVL game!

Yup
Likely also the majority view
Multiple people tell me how to do it
Perhaps itβs because I only use one blade & issues seen occur more with other blades
Here are data that can see canβt intubate is very uncommon with D-blade & the technique I use

Itβs not a recommendation by PUMA to use this technique, just offered as an example of a technique with which many of our group have had success. My experience is that variations in HAVL blade geometry havenβt had much impact so far - but havenβt tried some of the newer offerings
