Would you start CPAP on every late preterm baby born by cesarean, including the ones who look entirely well?
That is the premise being tested. Not CPAP for the babies who declare themselves, but 20 minutes at 5 to 6 centimeters of water for all of them, regardless of any respiratory distress. The question the investigators keep getting is why not more pressure, why not longer, and their answer borrows from pharmacology. If a dose has been shown to be safe and useful, you do not escalate it because escalating feels more protective.
Twenty minutes was not a compromise. It was the dose.
They are candid that plenty of clinicians remain unconvinced. A larger multicenter trial is now planned, for babies born between 35 and 36 weeks, with the goal of keeping these babies out of the NICU. If your unit waits for the grunting to start before reaching for CPAP, the premise of the PLaNT study may resonate with you ...

Have you ever grieved a patient who didn't die? Dr. Hailey Evans interviewed 41 neonatologists across Massachusetts and found something clinicians rarely say out loud: loss in the NICU isn't only about mortality. A baby who fails extubation, develops NEC, or survives with a serious morbidity can trigger the same grief response as a death. Twins where one survives. Families who fought through IVF only to face a hard outcome. These cases stay with people the same way a death does, even when everyone else moves on because "the baby lived."
Sometimes the baby survives and the loss still lands.
Most debriefs and M&Ms cover physiology and timeline, rarely the part where you go home replaying what you felt. Evans found that unsupportive culture, not the loss itself, is often what keeps clinicians from processing it. Worth sitting with before your next case review.

What if the outcome of a resuscitation depended less on the algorithm and more on who happened to walk into the room that day?
Dr. Charles Roehr describes neonatal resuscitation as a kind of lottery. Every mother baby dyad that comes through the door gets whoever is on shift, and every clinician brings a different read on acuity, a different comfort level, a different instinct for when to deviate from protocol. Guidelines can standardize a lot. They cannot standardize a person.
The algorithm ends where human judgment begins.
This is why simulation, clear roles, and a team culture where anyone can speak up matter just as much as your equipment. The next resuscitation you run will not look exactly like the last one, and Dr. Roehr argues that is not a flaw in the system. It is the reality we are training for.
