CT-based myosteatosis and muscle mass are increasingly studied in nephrology. How should these measures be interpreted? Physiological thresholds (age, sex, population) vs arbitrary historical cutoffs? A debate that strongly echoes GFR thresholds in CKD journals.lww.com/cjasn/citat…

Measuring GFR using CT urography performed as part of routine care is possible not only in healthy individuals, but also in patients with CKD. In this study, we demonstrate that: - CT-measured GFR is more accurate than GFR estimated from creat, cys C, or both (eGFRcr-cys).

- Hybrid GFR, defined as the mean between CT-mGFR and eGFRcr-cys would be even more accurate. These results open up new perspectives, particularly for patients with kidney cancer or urothelial carcinoma.

Very pleased to share this publication! doi.org/10.1016/j.ekir.2025.… A low eGFRcys/eGFRcr ratio is increasingly recognized as a risk factor for morbidity and mortality, including CV events. However, there was little or no data available on kidney transplantation.

In addition to diabetes and male gender, we identified myosteatosis and low muscle mass as predisposing factors. Visceral and subcutaneous fat mass were not determinants of the decreased ratio.

🧵1/9 Could we improve kidney care by rethinking who gets referred to a nephrologist? Our latest study from the SCREAM cohort suggests: yes — if we base it on predicted risk rather than fixed eGFR/ACR cutoffs. 📄 Caldinelli et al, 2025, @NDTsocial 🔗shorturl.at/0CzNq
