12 months old baby with gamma delta T-ALL. sCD3 vs CD7 shows the blasts are dim sCD3 compared with the reactive T cells. cCD3 vs CD5 shows the blasts are bright cCD3 and dim CD5 compared with the normal T cells. What translocation was found to be associated with this type?

One of the funnier mental imagery mnemonics that I created and stuck with me during AP/CP boards:
CLL/SLL cytogenetics = “The 7-11 Robbery at Midnight.”
🟢 del(13q) = the lottery winner (favorable prognosis)
🟡 Trisomy 12 = not great, not terrible (intermediate/atypical)
🔴 del(11q)/ATM = robbed of your ATM (adverse prognosis)
🔴 del(17p)/TP53 = the robber himself (high-risk disease)
I always pictured getting robbed at a 7-11 convenience store at midnight to remember that 11q and 17p are the cytogenetic findings you don't want to see.
Not a substitute for understanding the biology, but sometimes a ridiculous mental image is exactly what survives board prep.

There is no need to find in situ follicular neoplasia (so don’t stain every node) but if you do order stains, make sure you look at them carefully. ISFN often has brighter CD10/BCL2 expression. Patients may have FL in another node or may never develop FL so I don’t seek it out

CD markers very simplified!
CD34+ = Blast
CD3+ = T-cell
CD19+ = B-cell
CD33+ = Myeloid
CD34+ = Acute leukemia (but which type?)
CD34+ CD19+ = B-ALL
CD34+ CD3+ = T-ALL
CD34+ CD33+ = AML
CD34- CD33+ = APL (promyelocyte differentiated one step above blast)”
I've been looking into this weird inguinal/soft tx or lymph node case. IHCs CD3 CD20 CD5 ki67 CD10 were already done when it came in. Not specific. All in the right place. Thinking of syphilitic lymphadenitis? Haven't seen one. But I would appreciate opinions 🙏 @sanamloghavi
I don't really see these cases as often as I should. Can't help feel excited 😆

Chew your bones very well next time before you swallow. Got this from a sigmoid colon obstruction... Absolutely sharp point. Stabbed me in the finger..
Too bad, it was a few inches shy of exiting the rectum.

Soft tissue Rosai-Dorfman disease. The S100 protein stain is wonderful - it stains nuclei (white arrow) and cytoplasm in the abnormal histiocytes but not the lymphocytes that they have engulfed but not destroyed (emperipolesis).

I'm working on a teaching presentation on Hodgkin's lymphoma.
Picture below contains HRS-like cells from 9 different cases.
Which ones are classical Hodgkin's lymphoma and which ones are something else?

Quandary over atypia. How can you really tell? My tolerance has always been high but I've known someone who cries atypia at the slightest hint of uncomfy clearing.
What's your criteria? #gynepath
