Obstetric cardiac arrest
Let’s look at the big picture….
So among 355,000 obstetric anaesthetic interventions in a year in UK (delivered to approximately 50% of all births & at all times of day and night almost exclusively in the NHS)
Incidence of obstetric cardiac arrest
-Epidural for labour
1 in 56,500
- any anaesthesia for LSCS
1 in 8,500
-All epidurals & spinals
1 in 17,000
-all GAs
1 in 1,200
These fall into ‘big data’
They are no substitute for an RCT in assessing safety (except no such RCT exists or is likely to)
There will clearly be confounding by intention (the very highest risk cases more likely to have GA)
…..but they do provide welcome evidence of the absolute safety of obstetric epidurals& spinals (& possibly relative safety compared to GA…)
I think a credit to UK obstetric anaesthesia & wider surgical & midwifery teams.

Here’s a nice thing. The TV version of This Is Going To Hurt is all about the mental health of doctors, and culminates with the death by suicide of a junior doctor called Shruti (spoiler alert). There is a scene where a tree is planted in her memory, shot at Ealing Hospital. 1/3

Yesterday I shared an idea to support Ukraine by booking rooms for rent on AirBNB. 24 hours later, 100's of people are booking AirBnBs in Ukraine as a way to send immediate monetary assistance to people in hard-hit areas. The messages in response from the hosts are so moving

Wonderful seeing our anaesthetic registrars and fellows providing excellent care as part of the trauma team. Surgeons wouldn’t get very far without the whole theatre team

Latest data from @mbrrace @NPEU_UKOSS @BPSUtweet showing worsening outcomes from covid-19 amongst pregnant women and their babies. This underpins new JCVI recommendation that pregnant women are considered an at risk group. Vaccination strongly protective against severe disease.

Some personal thoughts:
Husband and I have just recovered (mostly) from Covid... no idea how we got it since we've been super careful and not done much. Shopping or swimming we think.
I am left grateful for masks & vaccines - here's why: 1/8
