Every cancer organ gets the same sequence: surgery is the standard, then trials show you often don't need it, then surgery becomes the salvage. Breast, larynx, limb, rectum, esophagus, mesothelioma. Bladder is next. Patients want their organs. 🧵

Reviewer 2 strikes again...🤦🏽‍♀️
In 1992 Peter Ratcliffe received this rejection from Nature. Almost 30 years later he won the Nobel Prize for the same discovery. Don't lose faith in the things you believe in.

#ASTRO2026 What if radioresistance in bladder cancer is dynamic, shaped by tumor-intrinsic biology, hypoxia and the TME? @achoud72 presented pre-clinical work challenging this paradigm: 🔹Acquired resistance implicated RAD51C, VTN & MYL9 🔹Hypoxia screens identified STAMBP & YAP1 as response modifiers 🔹 Hypoxic tumors showed substantial CNV burden, particularly on chr9 🔹3D tumor-like models are integrating matrix stiffness, tumor genomics & immune cells to better recreate RT response @UroToday @ASTRO_org

Pick any medical specialty other than your own. Who’s the most famous physician in it? You probably can’t name one. Almost none of us will be known outside our field, and even those who achieve real professional fame are mostly forgotten within a few decades. Fame is a poor reason to devote your life to medicine. The work itself matters more: caring for patients, discovering new knowledge, and teaching the next generation.

The GRAIL NHS Galleri randomized trial with over 142,000 participants was published today @NEJM "In this trial, we did not observe a lower incidence rate of stage III or IV cancers [the primary endpoint] across 12 prespecified cancers after three screening rounds with multicancer early-detection testing plus usual care than with usual care alone." nejm.org/doi/full/10.1056/NE…
