Women physicians get 25 percent more patient portal messages than men in the exact same practice, and 26 percent more from their own support staff. That is Dawn Sears's figure, and it is one of the quieter reasons she thinks women leave medicine. Sears, a gastroenterologist for 25 years, has seen it from both sides. She trained with no duty hours and no maternity leave, raised three kids, and served as program director and chief while working 0.8. Her starting point: "the system was designed for someone who is a male, who is married to a female, who is at home taking care of everything." Seventy to 85 percent of women physicians are moms, and many carry the elder care too. An article she read in May put the average age at which women physicians leave at 45. She takes on the obvious objection, that someone who plans to leave early should not apply at all. She had classmates who never practiced, and it bothered her. But "we thought we could do it all, and we were sold a pile of lies." She points to April 2020, when visits went from almost all in person to 80 percent virtual within four weeks. Health care can move fast when it has to. What she says works: Staff the inbox. If women get 25 percent more messages, give them 25 percent more MAs to sort the real medical questions from the venting. Ease the return. Three colleagues of hers asked in the past month for 50 percent of call and 50 percent of weekends for the first six months after a baby, and they got it. Add five days. Women need more screenings, and their children need well visits. Do the math. Losing her costs half a million to a million dollars, far more than the time she is asking for. And when a partner says it isn't fair, she asks whether they like their call schedule as it is, because if she quits in two months, their call is going to double. Next up, Team GI’s @TedHong9 discussing NRG-GI003 results of photons vs protons for HCC! Hypothesis: protons could reduce liver decomp and improve OS, the primary endpoint. This was based on data from preceding work published by @NiuSanford , showing potential gains with protons 💫

TORCH trial: TACE +/- ablation for intermediate stg HCC. IMO experimental premise doesn't make sense: treat incompletely w TACE (mRECIST ORR only 57%), ablate residual tumor, then attribute benefit to combo. Why not use more effective local tx upfront? & Role of IO for int HCC?

SBRT vs Microwave Ablation for Colorectal Liver Metastases LAVA-CRLM Phase II Trial | ESMO GI 2026 Key findings: • Randomized phase II trial in patients with 1–3 colorectal liver metastases (≤4 cm) • 1-year freedom from local progression: SBRT 83.0% vs MWA 77.7% (HR 0.87; P=0.70) • Significantly lower toxicity with SBRT: • Grade ≥2: 3% vs 25% • Grade ≥3: 0% vs 8% • One treatment-related death occurred in the MWA arm. Clinical takeaway: For carefully selected patients, SBRT provides comparable 1-year local tumor control with substantially lower toxicity, making it an attractive non-invasive local treatment option where expertise is available. New @JCO_ASCO * analysis of ~5,000 HCC patients treated with external beam radiation therapy across 30 centers in 11 countries. Median overall survival: -BCLC-0 (very early stage, a single tumor 2 cm or smaller): 6.8 years -BCLC-A (early stage, single tumor of any size or up to 3 tumors ≤3 cm each): 4.6 years -Treatment-naive BCLC-A: 5.4 years Retrospective, but the scale is striking. EBRT belongs in the frontline HCC discussion.

Wating for you @ #ESTRO26. Advocacy in Radiotherapy is not a choice. It is an act of responsability. Radiotherapy needs efforts to amplify voices: radiotherapy saves lives, it is effective and well tolerated but underrepresented by media and poorly understood. Speak up!

Bertolo & Develtere want cumulative incidence for RT counseling. Fine. ProtecT at 12 years: urinary leakage requiring pads 18-24% surgery vs 3-8% radiotherapy (Donovan, NEJM Evidence 2023). Applying their own framework to surgery produces numbers they’d never publish. Funny that.
Proud to present our new paper published in @JCO_ASCO , in which we look beyond the headline results and highlight how landmark prostate cancer trials provide valuable — and often underappreciated — high-level insights into late GU & GI toxicity! @RicBertolo ascopubs.org/doi/10.1200/JCO…
