Whenever I run an RPG in the "real world" (Vampire) I always abstract things culturally to keep players in the world + give myself historical leeway but having a 1988 West Hollywood hag PC means I've had to create a colorful cast of NPCs such as "Crown Guy" and "Wiggly AIDS Man"
