Fair points, nicely laid out.
With ADT, my top Q is: Does giving ADT now buy this patient sufficiently lower risk of something worse down the road?
If 6m ADT with MDT only delays need for ADT in the future, that’s not good enough. Needs to ⬇️death or ⬆️treatment-free survival.

Updated LUNAR analysis out in @EUplatinum. Now with a median follow-up of 32 months, exploratory analyses confirm the addition of 177Lu-PNT2002 to SBRT improves polymetastatic PFS (6 or more new lesion), castrate disease-free survival, and CT-based PFS (i.e., conventional MFS) authors.elsevier.com/sd/arti…

Very excited to welcome four outstanding new faculty to the Lank Center for Genitourinary Oncology @DanaFarber
1) Dr. Nazli Dizman @NazliDizman
2) Dr. Brian Russell @brian_russell39
3) Dr. Hamid Emamekhoo @HamidEmamekhoo
4) Pr. Neeraj Agarwal @neerajaiims
They bring complementary expertise spanning clinical investigation, microbiome science, informatics, quality improvement, education, and GU oncology.
Hamid and Neeraj will also take on important broader institutional leadership roles @DanaFarber and @Harvard Cancer Consortium @df_hcc
We are fortunate to have them—and the @DanaFarber_GU and our patients are in very good hands.
Welcome to @CityOfBoston and @DanaFarber!

📢 PALLIATIVE CARE IS SIMPLY GOOD CARE AIMED TO MEET PEOPLE’S NEEDS!
It is part of routine care.
It is NOT mutually exclusive with active treatment.
There is never “nothing we can do” - there is always something we can do to help.
It’s not always “find a new shiny drug.”

SENTRY is an IIT, phase II, for localized HR (N0 on PSMA). RT+18m ADT vs RT+6m ADT/ARPI.
RSI MRI and PSMA PET required. Focal boost required if feasible.
MDT alone encouraged for oligomet recurrence.
Primary endpoint: FFS; “failure” of initial Rx plan = salvage ADT or CRPC.

@DanaFarber and @DanaFarber_GU 's very own @AtishChoudhury presents compelling de-intensification data from A-DREAM/A032101 in mHSPC: 41% of favorable responders remained treatment-free with testosterone recovery 18 months after ADT + ARPI interruption, supporting treatment holidays as a promising strategy in selected pts.

📐 HR 0.68 = a 32% relative risk reduction in recurrence at any given moment during follow-up.
That sounds like a large benefit.
But the absolute numbers tell a different story:
BCG alone: 81.6% disease-free at 2 years
BCG + durvalumab: 86.5% disease-free at 2 years
Absolute difference: ~5%
Since BCG already works well, a big relative reduction maps to a modest absolute gain. @MaxKates @UrogerliMD @JoshMeeks

An important study that highlights the complexity of using PSMA PET as an endpoint.
Prognostic significance of PSMA tumor volume varied between patients with low and high volume of disease (*particularly relevant for #PSMA+BCR):
Clink link to article in European Urology. PSMA total tumour volume at 3months predicts overall survival independent of treatment in ENZAp. This raises important questions about how we should be measuring progression on PSMA. @ANZUPtrials @PCF_Science

Perioperative apalutamide + ADT vs. placebo + ADT with radical prostatectomy in HR localized or locally advanced PC
Our very own #DrMaryEllenTaplin from @DanaFarber_GU will take the stage at #ASCO26 and present phase 3 PROTEUS trial. This is a key multimodal treatment question and could shift the paradigm on how we think about systemic therapy in the perioperative setting. Dr Taplin has worked on countless Neoadjuvant Strategies in prostate cancer and it is rewarding to see her work materialize at the big stage !

📣🚨🚨Are we measuring the most meaningful endpoint? A question raised to the RadOnc community by @davidjeinstein
In the era of MDT and intermittent therapies, Treatment-Free Survival (TFS) may better capture real benefit.
