Every patient deserves the best chance at recovery. 💜 New research from JACS shows how much preventing complications can matter for frail patients undergoing pancreatic surgery. It’s a reminder that thoughtful care before and after surgery can make a difference.
Frailty is associated with higher mortality after pancreatectomy, but not necessarily because frail patients are less likely to survive complication.
In a national study of 71,104 patients undergoing pancreatectomy, postoperative complication explained 80.7% of the association between frailty and 30-day mortality.

I recently received a rejection comment from a leading international journal questioning whether the authors had sufficient “authority” to write a perspective on an emerging field in breast oncology.
What made the comment particularly striking was that the author group included a former President of Breast Surgery International, along with surgeons with established contributions to breast surgery, education, training, and global academic practice.
A strong scientific criticism of a perspective is entirely appropriate and welcome.
But journals should critique the evidence, reasoning and manuscript, not the perceived “authority,” standing or entitlement of authors to discuss a scientific topic.
For authors working in LMICs, such wording is especially concerning. Academic debate should be open to ideas and evidence, regardless of geography or institutional pedigree.
We always respect the editorial decision; we only hope the language of comments reflects the inclusiveness and scholarly standards expected of an international journal.
Global surgery cannot advance if “authority” is implicitly defined by geography, institutional pedigree, or personal procedural volume rather than scholarship.

Dr. Fahad Alam shared the results of his SORE (Simulation for Operating Room Ergonomics) Study. 70% of respondents reported pain while operating. The most common locations? The lumbar and cervical regions. And the impact doesn’t stop when the operation ends.

– Did I behave according to my principles?
– Did I treat the people with whom I interacted with in a friendly and considerate manner?

– Have I made myself a better person by cultivating my virtues?
