The media focuses on white skinned people. Children with darker skin are at higher risk from health inequalities partly due to poor health messaging. #ScarletFever

Scarlet fever is getting a lot of media attention at the moment, because of an increased number of cases and sporadic antibiotic shortages. I know parents are worrying. So, here's another set of photos of what it looks like on darker skin.

I looked at London Covid admissions today & they made me cry.
We've been admitting a medium sized hospital worth of Covid patients EVERY day for 3 weeks.
KCL hospital is one of London's largest at 950 beds.
On 6th Jan we admitted 977 Covid patients.
What have we done?

To explain our rationale...
COVID-19 seems to present as 2 phenotypes: initially high compliance with poor V/Q match and low recruitability, ‘dry’ lungs; versus subsequent low compliance, ‘wet’ lungs, potentially recruitable.

Intubatrd 1st covid 19 pt, SpO2 90-93 15 L. RR 22, talking sentences easily but followed advice RSI early. No BVM, held mask 10 PEEP. Sats 68 in 20s. Bagged up fine. Beware

Intubated a ?COVID pt today with an anticipated difficult airway + HD instability. Thoughts I've reflected on after the fact:
Before - role ID, plan for all contingencies (meds, rescue, resus), imagery of sequences, most experienced intubator, have filters/tubes/PPE ready

We're using simulation training to identify the problems around the intubation of patients with #coronavirus.
Pay particular attention to
1. Communication
2. Temp control
3. Access
4. Speed
