I think most thoracic surgeons and thoracic oncologist would welcome an apples to apples comparison of ChemoICI-surgery vs ChemoICI-XRT
Until then, patients deserve the treatment strategy that has been shown in this patient population to have the best outcomes
When speaking with patients we must share the data we have, not the data we think might be

Everyone: “AI is advancing so fast it may soon surpass human intelligence and end the world.”
Me: Wake me when it figures out how to get 180 people onto an airplane in under 40 minutes.
If the machines can solve Zone 7 standing in the aisle while Zone 1 is still boarding, then I’ll start worrying.
I think we have some time. @Delta

A growing body of evidence suggests millions of surgeries performed each year are unnecessary. They cause needless physical pain and financial stress—and are a terrible waste of resources

Make that THREE papers this week! 🚨
RAPTOR is out: 3,120 men & 11 RCTs. After RT alone, being BCR-free at 5y marked a functional-cure benchmark (8y DM 6.6%; 10y PCSM 2.5%). W/ ADT, late metastatic risk persisted.

Two papers in a week! 🚨
Now out in @IJROBP: after testosterone recovery following RT+ADT, faster PSA kinetics were associated w/ progression but the relationship with metastatic risk was inconsistent. redjournal.org/article/S0360…

Bladder preservation deserves a larger place in MIBC research. More randomized trials should be built around improving trimodality therapy, systemic integration, and long-term bladder-intact survival. What should we test next? @OncoAlert @OpenMedicineHQ @Uromigos @urotoday
New #bladdercancer #clinicaltrial opening now: Bladder-preservation for muscle-invasive #bladdercancer (MIBC) - #TroFuse055 is a Phase III clinical trial that evaluates the value of neoadjuvant sacituzumab tirumotecan (Sac-TMT) + MK3475A followed by consolidation MK3475A when combined with conventional chemoradiotherapy - this trial builds upon the value that chemoradiation brings to patients with MIBC who have elected to avoid radical cystectomy surgery - 1) Sac-TMT is a Trop2 binding antibody drug conjugate (ADC) conjugated to a potent topoisomerase-1 inhibitor payload (belotecan) and 2) MK3475A is subcutaneous pembrolizumab - Trial is opening globally and sponsored by @Merck - we are pleased to offer at our institution @AdventHealthCFL Cancer Institute Orlando, #Florida - Please note @BladderCancerUS @FLASCO_ORG @tompowles1 @DrYukselUrun @drenriquegrande @AndreaNecchi @shilpaonc - clinicaltrials.gov/study/NCT…

Disagree. During poster session it was packed house with way more attendees than normal.

Surgery or 5 sessions of radiation? For the first time, we have randomised head-to-head efficacy data. 🔥 #PACE_A at #ASTRO26 shows comparable 8-year cancer control, and nearly half of surgical patients still use pads at 5 years.
LBA 32 - EFFICACY OF RADICAL PROSTATECTOMY VERSUS STEREOTACTIC BODY RADIOTHERAPY (SBRT) FOR LOCALISED PROSTATE CANCER: RESULTS FROM AN INTERNATIONAL PHASE III RANDOMISED CONTROLLED TRIAL (PACE - A)
PACE-A (phase III) randomised 123 men with localised #ProstateCancer to 5-fraction SBRT☢️ (36.25 Gy) or prostatectomy, without ADT.
➡️At 8 years' median follow-up, biochemical/clinical failure-free rates were 91.2% vs 83.7% (HR 0.48; p=0.18), with 94.8% vs 94.1% at 5 years.
➡️Pad use at 5 years: 8.3% SBRT vs 48% surgery. Grade ≥2 toxicity was low in both arms.
✅SBRT offers comparable long-term cancer control with far better urinary continence.

As cancer treatment becomes more complex, new Accela technology is helping expand radiotherapy options with faster, more precise treatment planning and delivery. Learn more about this technology now found at @UHhospitals: ms.spr.ly/6016aYuRk
Importantly this is modern data that with a spacer gel and SBRT at 2 years 80% of men remain potent.
This is in a prospective RCT with patient reported outcomes.

Congrats again @Sophia_KamranMD on delivering a great discussant talk. Keep crushing it 💪
Disagreement isn’t disrespect, and different communication styles aren’t a crisis. If the people actually talking are fine, the audience can relax. Not every conversation needs a referee. Sometimes the real problem isn’t what was said…it’s the person determined to be offended by it.
Two brilliant GU physicians working toward the same goal. A hug and a photo. I really do love the GU oncology community. ❤️

@ASTRO_org Not trying to be controversial or contrarian, but I think the decision to move from an actual, interactive poster session to digital posters has missed the mark. #ASTRO26
Attendance is down.
Exchange of ideas is down.
And most importantly, opportunities for trainees to interact with investigators and others in the field are down.
Poster sessions have always been about far more than displaying data—they create conversations, connections, mentorship, and collaborations, especially for our trainees.
It’s not too late to fix this. Let’s bring back meaningful, in-person poster interaction.
