Is the next generation of KRAS G12C inhibitors changing the treatment landscape in advanced NSCLC 🫁 ? nejm.org/doi/full/10.1056/NE… Divarasib as Compared with Sotorasib or Adagrasib in KRAS G12C–Mutated NSCLC @FSkoulidis, @ShirishGadgeel, @DrSanjayPopat, @MDuruisseaux, @WadeIams, @JulienMazieres, @Decoster_Lore, @EnriquetaFelip, @mollylisc, @AkamatsuHiroaki, @k_cupp_, @MartinReck2, @bensolomon1 A new phase 3 trial puts divarasib head-to-head against existing 🧬 KRAS G12C inhibitors, with encouraging results. ➡️ In previously treated KRAS G12C-mutated advanced NSCLC, divarasib significantly improved median progression-free survival (9.8 vs 5.7 months; HR 0.58) and overall survival (17.7 vs 12.0 months; HR 0.56) compared with sotorasib or adagrasib. ➡️ Response rates were also higher (58.4% vs 32.3%), with fewer grade 3–4 treatment-related adverse events (24.6% vs 34.5%). #LungCancer #NSCLC Pinging Lung Cancer Faculty @peters_solange, @HenningWillers, @HHorinouchi, @BRicciutiMD, @PercyLeeMD, @bensolomon1, @Latinamd, @DrJNaidoo, @barlesi, @VanitaNoronha, @ChristianRolfo @Tony_Calles @s_couraud @marinagarassino @g_mountzios @BairdAM @barlesi @JFreemanDaily @Piuchagarrido @PercyLeeMD @bensolomon1 @VanitaNoronha @alexdrilon @JordiRemon @JulienMazieres @TerriConneran @HenningWillers

CARBOPLATIN IN EARLY TNBC | More Than Just pCR? A 2026 pooled analysis of 3 randomized trials (n = 1,084) offers important insights: 🔹 pCR: Improved with carboplatin (OR 1.89). 🔹 EFS: Significant benefit (HR 0.71) — 29% lower hazard of an EFS event. 🔹 OS: No statistically significant improvement (HR 0.93). 🔹 gBRCA1/2: Encouraging EFS signal (HR 0.50; P = .05). 🔹 Biomarkers: None validated to predict additional carboplatin benefit. Take-home: Carboplatin improves pCR and EFS, but an OS benefit remains unproven. These findings come from the pre-immunotherapy era. 📖 Felsheim et al. Would you like a glimpse of the future? Where solid tumor oncology may be heading? Look no further. Agenus may have had some ups and downs in the past (BATMAN -> ROBBIN), but Bot Bal looks like it has the capacity to absolutely change how we manage MSS solid tumors. UNICORN, now in Nature Medicine. Phase 2, non-randomized, window-of-opportunity. Resectable rT3-4 colon cancer. One dose of botensilimab, two doses of balstilimab, surgery around day 30-40. Highlights: MPR 36% (5/14) in MSS disease, pCR 29%. Omg. (compare to FOLFOX in FOxTROT: 8%) Bot alone in MSS: 0%. The PD-1 matters. dMMR Bot Bal: MPR 100%, pCR 93% 😍12-month EFS 100% in both Bot Bal cohorts Yes there is tox. irAEs in 54%, mostly flu-like symptoms, and only one grade 3 (hyperthyroidism). No grade 3 diarrhea. Every MSS responder cleared ctDNA before surgery, that’s a helpful tool for predicting response in a disease that may fibrose and stay visible on imaging. The holy grail is almost here: melting the majority of rectal cancers away. In MSI-H we have AZUR-1. In MSS, perhaps we have this in the future. nature.com/articles/s41591-0… @OncoAlert @TheGutOncLab @Onco_Nexus

🚨 Is immunotherapy finally breaking the pMMR colon cancer barrier? The phase II UNICORN trial, published in Nature Medicine, reports striking pathological responses with neoadjuvant botensilimab + balstilimab. 🔬 56 patients | Resectable T3–4 colon cancer Major pathological response (≤10% viable tumor): 🔹 pMMR colon cancer * Botensilimab alone: 0% * BOT + BAL: 36% * pCR with combination: 29% 🔹 dMMR colon cancer * Botensilimab alone: 36% * BOT + BAL: 100% * pCR with combination: 93% (13/14) 🔥 The most intriguing finding? Even traditionally immunotherapy-resistant pMMR tumors demonstrated meaningful pathological regression with Fc-enhanced CTLA-4 blockade plus PD-1 inhibition. 📊 12-month EFS: 100% in both combination cohorts, although follow-up remains immature. ⚠️ Important limitations: Small, nonrandomized cohorts (n=14 each), pathological response as the primary endpoint, and no definitive survival comparison. 💡 My take: The 93% pCR in dMMR is remarkable, but the 36% major pathological response in pMMR may be the more biologically provocative signal. Could next-generation CTLA-4 antibodies finally expand immunotherapy beyond dMMR colon cancer? @oncoalert @NatureMedicine #ColonCancer #Immunotherapy #UNICORNTrial Full paper link in comment below ⬇️

Great Results from DeLLphi-305 🫁 however with CRS in nearly half of patients, this means readiness is the real challenge now. Monitoring pathways, step-up dosing logistics and training community teams will decide how quickly this reaches patients outside academic centres. #LungCancer
DeLLphi-305🚨 A 0.42 hazard ratio for death in extensive-stage small-cell lung cancer. In a disease where we’ve celebrated gains of two to three months, this is a different level. 🫁 Tarlatamab–Durvalumab Maintenance for Extensive-Stage Small-Cell Lung Cancer nejm.org/doi/full/10.1056/NE… @MP_3855, @ADesaiMD 📊 In the open-label phase 3 DeLLphi-305 trial, 563 patients with extensive-stage SCLC without progression after first-line chemoimmunotherapy were randomized to tarlatamab plus durvalumab or durvalumab alone as maintenance. The median overall survival was 21.1 versus 12.7 months (HR 0.42), and the median PFS 7.4 versus 1.9 months (HR 0.34). Grade ≥3 adverse events occurred in 46.8% versus 40.6%; cytokine release syndrome, mostly grade 1–2, affected nearly half of tarlatamab-treated patients. #LungCancer Pinging our faculty @christine_lovly, @GlopesMd, @DrSanjayPopat, @StephenVLiu, @jillfeldman4, @UOzkerim, @DrMirallas, @MartinReck2 , @bmassutis @UmbertoMalapel1 , @CharuAggarwalMD @peters_solange, @HenningWillers, @HHorinouchi, @BRicciutiMD, @PercyLeeMD, @bensolomon1, @Latinamd, @DrJNaidoo, @barlesi, @VanitaNoronha, @ChristianRolfo @Tony_Calles @s_couraud @marinagarassino @g_mountzios @BairdAM @barlesi @JFreemanDaily @Piuchagarrido @PercyLeeMD @bensolomon1 @VanitaNoronha @alexdrilon @JordiRemon @JulienMazieres @TerriConneran @HenningWillers
