I'd love 'Failed discharge' to go the way of 'No rehab potential' If someone towards the end of their #Last1000days gets home to loved ones for even a few of them, that's a success not a failed discharge. Instead, as @mancunianmedic so rightly says, it's an emergency readmission

I hate seeing the words “failed discharge”. This morning it is all over notes where so much time, care, energy and effort was invested. Two words undermine a full MDTs hard work 😓
