🇮🇹 TOWARDS MORE TARGETED PROSTATE CANCER SCREENING
Prostate cancer screening is being reconsidered in Italy and across Europe, following decades of debate over the limitations of prostate-specific antigen (PSA) testing.
Introduced in the 1980s, PSA-based screening enabled early cancer detection but also identified many cancers unlikely to become aggressive or life-threatening. Concerns about overdiagnosis and unnecessary treatment eventually led to the discontinuation of earlier screening practices based on PSA testing alone.
🎗️ In this video interview, Silvia Deandrea, Senior Expert/Researcher at ATS Pavia and EUCanScreen Task 6.1 Leader, discusses Italian and European pilot projects, including initiatives coordinated through the Joint Action, that are exploring new screening protocols.
Moving beyond the indiscriminate use of PSA testing, these protocols combine PSA results with urological assessment, magnetic resonance imaging (MRI) when necessary, and an evaluation of the risk of disease progression to better identify cancers requiring treatment.
A new approach to PSA screening, designed to reduce overdiagnosis without losing the benefits of early detection.

🇮🇹 SUPPORTING INFORMED CHOICE THROUGH RISK-BASED SCREENING COMMUNICATION
Deciding whether to take part in cancer screening requires access to complete, balanced and evidence-based information. How that information is presented, and how well it responds to people's needs, can make a difference.
In this video interview, Cinzia Colombo, Head of Unit at the Istituto di Ricerche Farmacologiche Mario Negri IRCCS (@MarioNegriIRCCS) and EUCanScreen Task 9.1 Leader, discusses how screening communication can respond to different information needs and support individual decision-making.
🎗️ As part of EUCanScreen, researchers are reviewing scientific evidence on effective communication approaches and preparing focus groups involving people from different European countries. The aim is to explore their information needs, perceptions of risk-based screening and willingness to participate.
The results of these activities will provide a basis for multidisciplinary discussions on best practices in screening communication.
Informed screening decisions begin with information people can understand, evaluate and trust.

💛 For the Public
Same disease. Same continent. Different odds.
1 in 11 women in the EU will develop breast cancer before the age of 74. Yet breast cancer mortality rates differ around twofold between EU countries, and access to organised screening is one of the reasons why.
The EU Council Recommendation on cancer screening now calls for organised mammography screening for women aged 45 to 74. On World Breast Cancer Day, EUCanScreen is working with partners across Europe to make that recommendation a reality for every woman, wherever she lives.
Your postcode should not decide your chances. 💜

🩺 For screening professionals:
🇮🇹 THERE’S A TIME FOR EVERYTHING. THE TIME FOR PREVENTION IS NOW!
In the Autonomous Province of Trento (@ProvinciaTrento), two experiences in colorectal and cervical cancer screening show how organised programmes can strengthen prevention and continuity of care through structured pathways, timely follow-up and evidence-based approaches
In colorectal cancer screening, people with a positive faecal occult blood test (FOBT) receive structured telephone counselling from healthcare professionals, connecting screening with diagnostic follow-up. The programme reports 95% adherence to colonoscopy (compared to a national average of 83.7%), with 92% of people receiving counselling within 6 days of a positive test.
🎗️ In cervical cancer screening, the introduction of primary HPV testing enabled the programme to compare women undergoing their first HPV test with those returning for a subsequent screening round. HPV positivity decreased from 8% at the first round to 3% at the subsequent round, alongside lower Pap test positivity at triage and fewer high-grade cervical lesions (CIN2+) detected.
Two different screening pathways, one shared goal: turning organised prevention into timely, effective care.

🔬 For researchers & innovators:
🇮🇹 USING DATA TO RETHINK COLORECTAL CANCER SCREENING
Making better use of information collected through colorectal cancer screening could help identify different levels of individual risk and guide the development of more targeted screening protocols.
In this video interview, Carlo Senore, Regional Screening Coordinator, @CpoPiemonte, and Task Leader of EUCanScreen Tasks 5.3, 9.4 and 10.4, explains how researchers are investigating the use of results from earlier colorectal cancer screening rounds, alongside information on lifestyle and environmental exposures, to better assess individual risk.
🎗️ While screening programmes already collect valuable data, gathering additional information directly from participants introduces a further challenge: the time and effort required could discourage participation, with potential implications for equity and access.
A study currently underway in Italy, funded by the Ministry of Health, is examining the impact of lifestyle questionnaires on screening participation, with the aim of informing future communication and invitation strategies.
Better screening strategies depend not only on the data we collect, but also on how we collect them.

🇮🇹 BRINGING CERVICAL CANCER PREVENTION CLOSER TO WOMEN.
In Taranto, Italy, ASL Taranto is working to re-engage women who have never participated in organised cervical cancer screening, either because they did not receive an invitation or did not attend their scheduled appointment.
Reaching these women is a public health priority, given the crucial role of early detection in preventing cervical cancer and reducing its incidence.
🎗️ To strengthen cervical cancer prevention, ASL Taranto has launched a public awareness campaign promoting cervical cancer screening and HPV vaccination as part of an integrated prevention strategy. The initiative includes additional screening sessions at local Community Health Centres, alongside dedicated HPV Vaccination Open Days offering free vaccination to unvaccinated women born between 1989 and 2001.
The campaign also involves general practitioners, pharmacies, municipalities and gynaecologists to promote a culture of prevention, raise awareness of available services and facilitate access to cervical cancer screening and HPV vaccination.
Reaching more women starts with bringing prevention into their communities.

🇮🇹 RISK-STRATIFIED SCREENING: ITALY’S EXPERIENCE IN EUCANSCREEN
Italy is bringing one of the world’s first experiences of individual risk-stratified cervical cancer screening to EUCanScreen.
By adapting screening pathways according to whether and when women were vaccinated against HPV, Italy has introduced a differentiated approach that takes individual risk into account.
🎗️ Through EUCanScreen Work Package 9, this experience can be evaluated and shared with other European countries. At the same time, Italy is learning from other risk stratification strategies developed across Europe, including those based on HPV genotyping, which are expected to be introduced into Italian screening programmes in the coming years.
In this video interview, @giorgi_rossi , Director of the Epidemiology Service at @Ausl_RE and Scientific Coordinator of EUCanScreen Tasks 9.2 and 9.3, outlines Italy’s experience in risk-stratified cervical cancer screening and highlights the value of this two-way exchange of knowledge and experience.

🇮🇹 STRENGTHENING CANCER PREVENTION THROUGH EUROPEAN COLLABORATION
Personalised cancer prevention requires more than scientific evidence: it demands connecting research, expertise, and European initiatives.
Within EUCanScreen WP9 (Risk-based approaches), Università Cattolica del Sacro Cuore (UCSC) contributes to developing guidance on integrating risk-based approaches into organised cancer screening programmes, examining effectiveness, implementation requirements, and equity implications.
To maximise impact, this work relies on continuous dialogue across Europe. At the EUCanScreen Consortium Meeting in Turin (2025), UCSC highlighted synergies with PROPHET, focused on personalised prevention, and ECHoS, dedicated to European cancer ecosystem networks. The dialogue continued at the ECHoS Cancer Mission Fair in Warsaw (2025), where EUCanScreen brought its perspective on organised screening and risk stratification to discussions on prevention and early detection. Most recently, at the PROPHET event in Rome (2026), UCSC engaged with stakeholders on the Strategic Research and Innovation Agenda (SRIA) and the European Roadmap on Personalised Prevention.
🎗️ These connections extend further through @jane_eu_ja (JANE-2), where the UCSC/Fondazione Policlinico Universitario Agostino Gemelli IRCCS team coordinates the Network of Expertise on Personalised Prevention, and a dedicated subgroup within the European Commission’s EU Health Policy Platform involving EUCanScreen, EUnetCCC, JA PCM, and ECHoS.
The ultimate objective of these synergies goes beyond individual projects: connecting complementary expertise, reducing fragmentation, and transferring innovations to make risk-based cancer screening more effective, accessible, sustainable, and equitable across European healthcare systems.

🇮🇹 TOWARDS RISK-STRATIFIED CANCER SCREENING IN EUROPE.
Adapting cancer screening programmes to individual risk profiles could help make prevention pathways more targeted and responsive to people’s needs.
In the interview “Italy and the introduction of risk-stratified screening programmes”, Paola Mantellini, Director of the Cancer Screening and Secondary Prevention Unit at the Institute for Cancer Research, Prevention and Clinical Network (ISPRO), and Scientific Coordinator of EUCanScreen’s WP9, discusses Italy’s contribution to the development of risk-stratified cancer screening.
With 12 Italian institutions participating in the Joint Action, Italy is working alongside European partners to assess scientific evidence and identify criteria that could support the introduction of risk stratification at a local level.
🎗️ Focusing on breast, colorectal and cervical cancer screening, this work involves examining the available scientific evidence to determine whether existing programmes could be organised according to individual risk. This could mean more frequent invitations and new or complementary tests for people at higher risk, while those at lower risk could be offered longer screening intervals and less intensive interventions.
The activities also extend to developing guidance for lung, prostate and gastric cancer screening, considering specific risk groups and geographical contexts.
From scientific evidence to practical guidance, EUCanScreen is exploring how cancer screening programmes can better respond to different risk profiles.

🇮🇹 Engagement as a practice.
Across Italy, several initiatives have proven the value of actively involving stakeholders in cancer screening. The strength of this approach lies in the diverse perspectives each and every contributor offers. Stakeholder Forums create a space where citizens, patients, clinicians, public health experts, and communicators can dialogue around the same table. In the recent Stakeholder Forum on prostate cancer screening, 24 key actors collaborated through 2 Delphi consultations, 7 plenary meetings, and targeted literature reviews.
🎗️ This collective effort identified 16 priority questions across four key areas: data on the usage of PSA testing, recruitment methods, and pathway redesign; screening protocol; communication, training, and co-design; ethical, legal, social, and monitoring aspects. 🏛️ For policymakers & health authorities:
A SYSTEM BUILT ON A NETWORK.
In Italy, organised cancer screening operates within a decentralised health system across national, regional, and local levels. Different responsibilities, one shared goal: ensuring coordinated and consistent screening programmes across the country. Across these levels, different bodies work in close connection, with continuous feedback between them. At national level, the Ministry of Health sets the overall framework, long-term objectives and health benefits package, and monitors the system. Regions govern their health systems, adapting national recommendations to regional needs and defining organisational frameworks and resource allocation. Local Health Authorities deliver services, including preventive and public health care.
🎗️ Within this network, the National Centre for Screening Monitoring (ONS – Osservatorio Nazionale Screening) provides technical support to the Ministry of Health and the Regions. With the Institute for Cancer Research, Prevention and Clinical Network (ISPRO) as its operational and administrative hub, the ONS connects regional screening coordination centres and works closely with the National Institute of Health, scientific societies, and citizens' and patients' associations. This network turns collaboration into concrete action: monitoring screening performance, implementing information systems, producing national guidelines, ensuring quality assurance, and driving research.
Through EUCanScreen, practices developed within Italian screening programmes can be shared at European level, while approaches and experiences developed with European partners can be transferred, tested and adapted in the Italian context. This ensures an effective exchange in both directions.
