An excellent start to the Complex Mapping and Ablation Workshop at Karolinska University Hospital in Stockholm, with complex VT cases and practical insights shared throughout the day. Directed by Dr Finn Akerstrom (@finnakerstrom) with K’s great team, Dr Peter Karl Jacobsen and Dr Javier Moreno

#Epeeps, any suggestions on the mechanisms for this? Concealed anterograde/retrograde LBB conduction?

Interesting!
The TRAVERSE trial shows reduced risk of acute brain lesions with transeptal approach in patients undergoing left ventricular catheter ablation, compared to a retrograde aortic approach. ahajournals.org/doi/full/10.…

This is in line with our workflow. With meticulous ICE guided mapping and ablation in aneurysmal ARVC (especially inferior aneurysm), epicardial access is usually not necessary. Without ICE this is much more difficult imo and one often has to go epi to compensate. Agree #epeeps?

I'm referring to first'time ablation.

4th time inferoposterior (by some called posterolateral) concealed AP. This quite nicely shows how LV pace (pic1) allows for easier mapping (annotation on "A" on MAP 1-2). During RVA pace a larger area of VA continuity is seen. Examples are at successful site of ablation. #Epeeps

Trigger mapping proof-of-concept: pxAF with 3 PVI scheduled for a non-PV trigger mapping protocol. Surprisingly RSPV still reconnected! Just to be sure we completed the protocol anyway. AF induction from RSPV: pic1, FF-NF reversal with AF start in first Grid in. #Epeeps

Roof dependent flutter around left veins with BIG jump over the posterior wall via the septopupmonary bundle. Green dots = good PPI. Patient with a scarred LA and previous anterior mitral + roof line. #Epeeps

WCT with reset by septal refractory PAC delivered from Abl positioned at superoposterior MA (pic1&2). Baseline ECG (pic3) and tachy start (pic4). What's the diagnosis #Epeeps?

Agree. I think reset of preexcited AT would give a variation in VA interval though. In this case, tachycardia was not indicible after ablation of the AP.
