🚨 SEVERE HAIR LOSS → THIS MUCH CHANGE. These photos are labelled 2 and 14 months on HRT. Their reported treatment history included estradiol, spironolactone, progesterone, oral finasteride followed by dutasteride, oral + topical minoxidil, and microneedling. In androgenetic alopecia, DHT activates androgen receptors in genetically susceptible follicles, gradually driving them to produce smaller, finer hairs. Reducing that androgen signal can help some follicles recover. One approach is to inhibit 5α-reductase, the enzyme that converts testosterone into DHT. Finasteride and dutasteride work here. Reducing testosterone also reduces the raw material available to make DHT. But suppressing testosterone is an UNFAVORABLE trade-off for hair loss alone when those broader hormonal changes aren’t wanted. Another approach is to block the androgen receptor. Investigational GT20029 takes a different approach: getting cells to break down the receptor protein itself. That removes the protein, but doesn’t permanently delete its gene. Estrogens may also help support anagen, the active growth phase, although their direct effects on human scalp follicles are complex. This case cannot tell us how much estrogen contributed. The research opportunity: an estrogen treatment that stays active mainly in the scalp but rapidly becomes inactive once it reaches the bloodstream. If proven safe and effective, that could help a lot of people while limiting hormonal effects elsewhere. Their starting hair loss was severe, so complete restoration wasn’t something to count on. There’s substantial visible improvement, but frontal recession remains. Meaningful partial recovery can still matter, and these photos don’t establish their final regrowth limit.
