🧵A healthy 35-year-old male 🙋♂️presented himself to the emergency department for worsening palpitations and chest discomfort. EKG showed diffuse symmetric T wave inversions and possible epsilon waves in right precordial leads. Echocardiography was immediately performed ⬇️
The presumptive diagnosis of the emergency department was later confirmed by cardiac magnetic resonance and genetic testing 🧬 Diagnosis: arrhythmogenic Right Ventricular Cardiomyopathy (ARVC) with mutation in plakophilin-2 gene #echofirst #arvc #epsilonwaves #echocardiography
✔️ If the shock is adequate and there is no correctable cause (e.g. hypokalemia) - the patient must be monitored, if the shock recurs - antiarrhythmic therapy can be helpful until the morning comes (e.g. amiodarone 150-300mg if QTc ok) 🌅
✔️ If the shock is inadequate/ electric storm ⛈ - the magnet on ICD will stop the defibrillation 🧲 But only if the patient is monitored! 🖥
