No. Socrates never said any of this. He left no writings. What we know comes from Plato, Xenophon and others. This list of “symptoms” appears nowhere in those sources. It is a modern social-media meme, not an ancient text. Plato’s Republic does discuss how regimes decline, but in a very different way.
