Way too much counterfeit supplements on Amazon. But you like the pricing... What I prefer about Fullscript is the direct to brand sourcing model - so you know what you are getting. My Fullscript is open to everyone with exclusive discounts... us.fullscript.com/s/reallyop…

My wife isn't a fan of my interior design. I told her she's wrong. She's wrong, right?

He's right. And his comment wasn't all that deep or controversial within the context of his work. The post was also a bit of an IQ litmus test for those who can't process basic context and instead jump to emotional reactivity.

Dumped Amex Platinum last year. Garbage product. The airport lounges are awful. We hit up a regular airport bar/restaurant or even just an empty gate to chill at. It's infinitely better than the putting your name on a waitlist for access to the Amex homeless shelter.
I’m done with @AmericanExpress platinum card. It was worth $900 a year when you could get into lounges with it. There was some exclusivity. Now every low class reject gets in. An hour waitlist and then when you finally get in, it’s people on speakerphone, screaming, acting like it’s a Carnival cruise. Amex has been enshittified. I’ll save my 895 bucks a year.

Pessimism contributes to death from heart disease? Ahhhh…. That’s why @realDaveFeldman will outlive us all

What gives you pleasure today will give you pain tomorrow. The third drink is all pleasure at midnight. By 7 AM it is all pain. Pleasure and pain come as a pair. Nobody gets to buy just one side. This is why you don't chase pleasure. You'll always pay for it.

Exit the insurance madness and win. Not just patients doing it with alternatives (check my page), but physicians taking back autonomy as well.
Banner day @KaufCare … Wow Growing steadily and today felt like a full-on ER shift. - cyst removal - massive facial lac to bone - IV meds with complex presentation - X-rays - several chill cases - US r/o DVT AWESOME. Volume ✅ Revenue ✅ Absolutely in love with practicing medicine again. Nothing stops this train. …And to cap it off, a patient emailed me one of the most profound and meaningful thank-you emails of my career. This is the way we were meant to practice medicine. Period.

Pessimism can kill you. But that's not the full story. Finland followed 2,267 adults for 11 years. The most pessimistic had 2.2 times the risk of dying from coronary heart disease as the least pessimistic. But optimism did nothing. No protective effect at all. Which means that the fix is not forced positivity. It is dropping the expectation that everything will go wrong.

When hungry, we eat. When tired, we sleep. Nobody tries to fight sleep every day. It would be miserable, and you would eventually lose. We tell people to fight hunger every day and call it discipline. Yet willpower always loses to biology.

Not sure we quite understand what "whole foods" is - but it could certainly be worse.

We are already seeing this abundance, though. It's already here. Upload your medical records to your favorite AI and you can already skip expensive doctor visits in many cases. It already reads imaging. You can upload extensive health records and get incredibly useful feedback. For free. This is already happening. More specialized integrations will be expensive at first because they can be, just like a new drug, but it's only a matter of time before they are all fully disrupted.
AI snake oil men…. keep promising some magical abundance while ignoring economics, the government, and human behavior. Cheaper to produce doesn’t mean anyone will charge less. And if government pays a hospital more for an AI add-on than the physician actually reading the scan, that tells how how screwed up government price setting is.

Trashing optimization is becoming trendy now. Entirely predictable. Anything popular brings out the contrarians. Human nature. Their whole argument is pointing at people who are clearly not optimized and calling that optimization culture. If obsessing over optimization is wrecking your mental health and your life, you are not optimized, by definition. And definitely not really optimized.

These numbers should embarrass the whole medical establishment. Microsoft built a panel of AI agents that argue about your case, order tests one at a time, and settle on a diagnosis. On 304 NEJM cases so hard they get presented at conferences, the panel hit 83%. Human generalists got 36%. ER cases: 80% vs 47%. Half the testing cost. And it lifted weak models the most, which tells you the magic is in the process, not the model. The future of medicine is AI.

"Vacation" is a funny word. We have normalized it so completely that nobody stops to ask what it actually is. A quota of trips per year, dosed out like a vitamin you are deficient in. For what? Why? Who decided that going somewhere is a requirement of life? And to be clear, rest is not the issue. Time off is not the issue. I am talking about the vacation itself. The production.

You can apply this to almost everything that AI is going to disrupt. Engineering, coding, medical, driving, even sales and customer service. The humans are the weak link and always will have been. But our collective hubris make this a difficult reality to accept.
To anyone that said agents write slop, sorry, the data is in, it has been humans this whole time. Now we need the agents to clean up this mess.

Centenarians have unique genes - which is the actual secret - though these are always humorous. Much of the longevity research centers around unlocking these genetic differences and exploring how we can apply them to everyone. Centenarian research is super interesting.

This probably is the most interesting story that came about from the KETO-CTA study. The rabid mobilization of an opposition party that had no interest in nuance or accuracy. And we still see it today with @Drlipid absurd "breaking" news that is old news and framed inaccurate.
Right. It's actually one of the most incredible stories of our research. There's a very strong resistance by critics to openly discussing the anomalous dataset that was provided to us from Keto-CTA, leading to the retraction of that paper. We're in talks right now on a successor to our documentary and this would definitely be a central theme... (It's a bit like a real life Emperor's New Clothes)
