Goodness me, if you read one thing on health care today read this. And multiply it several fold for Northern Ireland…. A perfect case of how to get it so wrong (less than 5% spent in GP, over 8% would feel like a luxury)
The healthy subsidise the sick. The occasional patient subsidises the frequent attender. That’s how pooled healthcare works, and… … it works best when not everybody needs to knock on the door at once. Except, more people are knocking, more often, myemail.constantcontact.com/…

So for all your GP services in NI we spend just over 50p per person per day (4.7% of the health budget) AXA health insurance starts from £1.49 and how often would that be called on? This is why access remains a problem.

At a time when we need to ‘shift left’ and move focus to out of hospital care, the percentage of total health spend on GP has significantly decreased rather than increased!!!. Someone make it make sense….
Commenting on the latest GMS stats GP chair Dr Frances O’Hagan said, “These latest figures confirm what GPs and patients already know, general practice in Northern Ireland is under sustained and growing pressure." Full statement here > brnw.ch/21x35Av

“Increase capacity and strengthen General practice; Everything else is a band aid” This is really worth a watch esp in relation to patient outcomes and acute flow. Great advice @DrLKVaughan ⬇️

“76% of doctors were able to return to practice within 12m” Whether person centred or system focused this vital service cannot be delayed any further. Achieving equality matters for NI NHS in more ways than one.
End the inequality. Extend NHS Practitioner Health to Northern Ireland. Doctors in Distress and a coalition of leading medical organisations are calling on the UK Chief Medical Officers to around 5,200 doctors and dentists who do not have services available elsewhere in the UK.

Thanks to @DoctorsDistress and all the co- signatories on this letter. Proud to have supported this, and hoping we in NI will finally gain access to this vital service

The message and intent is correct here. It is effectively a properly resourced and staffed General Practice. It’s pretty much what we should do and want to do if only we could….

Huge thanks to the 4 GPs & Harland Practice team who came together on Saturday to run a super clinic to provide care for 80 of the longest waiting Gynae patients from HSC Trust waiting lists. @marktaylor16 @mcentee_siobhan @AlanStout19 @healthdpt @rcgp_ni @mikenesbittni

⚠️🆘️GP WARNING: 'Young people have died because they refused to wait in A&E. It’s so sad, it shouldn’t be happening.' 🎥🎤Chief Reporter @jamesgould23 speaking to West Belfast GP Dr Michael McKenna. He says a number of people from his practice have died because they didn’t want to face long waits in emergency departments. He detailed how more and more people are staying away from A&E, even when they urgently need care. He also made clear that the voices of GPs need to be heard. It comes after a shocking new report highlights a “catastrophe” unfolding in Northern Ireland’s emergency departments. It shows the number of 12-hour-plus waits and associated excess deaths, has more than doubled over five years. The report by @RCollEM states that, in 2025, there were 1,032 excess deaths linked to waits of 12 hours or more in EDs while awaiting admission. The body’s State of Emergency Medicine in Northern Ireland report found that, while the figure for 2025 is slightly lower than in 2024 (1,122) and 2023 (1,063), the estimated mortality rate has still more than doubled over five years.

Starting from a pitiful 5.4% of the spend in Northern Ireland. No wonder GPs are on their knees here and the crisis continues yet no action on an ask of just 1% last year. A minimum investment guarantee would be a lifeline but it’s a long way to 15% and time is running out
General practice now accounts for 8.3% of NHS spend, the lowest in a decade. hsj.co.uk/primary-care/gp-sp… As we work towards shifting care into the community, resources must follow. A clear commitment to both increasing investment in general practice and publishing transparent, annual data on general practice and primary care funding is needed to demonstrate progress and build confidence in this direction. @HSJnews @DrSelvarajah @doctor_katie @DrSteveTaylor

Another essential read from @dave_dlt The march towards transactional care and taskification of general practice continues Protecting GP continuity within communities and neighbourhoods should be a priority lest we end up here!

It is unacceptable that healthcare workers are being excluded from life saving help solely because of where they live. Dr Adam Kay, our Patron, joined leaders in Northern Ireland last week to call for NI staff to have NHS Practitioner Health services.

Too true @AlanStout19 Patients in NI are getting the access that 5% of the budget delivers when closer to 10% is what’s required. Without a clear focus on incremental investment, anything else is merely skirting around the edges of taking action.
We heavily front load our practice on Mondays and Tuesdays (our busiest days by far) with lots of GPs and lots of staff. It works and we can provide a good service. We simply can’t afford to do it all week and we struggle It’s nothing more complicated than resource=staff=access

“Continuity is not nostalgia it’s a clinical tool” 👏👏👏 @dave_dlt “If we can’t rebuild continuity everywhere overnight, the least we can do is name the problem honestly” I’d take preservation of what we have and focus on rebuilding if on offer today! Such is its importance
📝 Chronic illness isn’t lived in appointments - it’s lived in the gaps between them. In healthcare we meet people in fleeting moments; for patients and families the story keeps running, day and night. 👇👇👇👇 open.substack.com/pub/davetr…

Completely agree @DannyDonnelly1 and that evidence points to lives saved. The longer this stays on the to do list and not actioned, the greater the harm
The Chief Medical Officer today told the Health Committee that the evidence for benefits associated with introducing minimum unit pricing for alcohol in NI irrefutable. If not brought in his will be a huge missed opportunity to save lives, reduce harm from alcohol. #HarmReduction

I’ve never understood why some people take issue with NHS doctors who work part-time. How many hours a doctor works each week is their choice – as it is for anyone else – and must fit in with any other personal, family, caring or professional commitments they have.
