This is Wonderful demonstration of "Intersphincteric resection for ultra low Rectal Cancer" and followed by Recto-anal Anastomosis by Robotic surgery. Intersphincteric resection (ISR) is indicated for carefully selected ultra-low rectal cancers located within 1–5 cm of the anal verge, where conventional low anterior resection cannot achieve a safe distal margin while preserving the sphincter complex. The procedure involves precise dissection in the intersphincteric plane between the internal and external anal sphincters, enabling complete mesorectal excision, adequate distal clearance, and restoration of continuity via coloanal anastomosis often stapled rather than abdominoperineal resection with permanent colostomy. Its importance lies in combining oncologic radicality with sphincter preservation, thereby improving quality of life by avoiding a permanent stoma in appropriate patients, provided neoadjuvant therapy, favorable tumor biology, and adequate sphincter function are present. Robotic assistance further enhances visualization and instrument dexterity within the confined pelvis, facilitating meticulous plane dissection and reducing conversion rates.

🦅Have you watched the video yet? EAGLE-2 (published in @bjsurgery) tested safe-anastomosis training in real-world practice across 60 countries. The result? Training was associated with: ✔️ Fewer anastomotic leaks ✔️ Fewer reoperations ✔️ Fewer readmissions And importantly, benefits appeared to persist even when some surgeons had completed the training two years earlier. 🎥 Watch Professor Dion Morton and Elizabeth Li discuss the study and the potential for scalable digital education to improve colorectal surgery worldwide: bjsacademy.com/bjs-academy/c… #EAGLE2 #Surgery #ColorectalSurgery #MedicalEducation #SurgicalInnovation

TESAR Trial in 2 Slides: Completion TME vs. Organ Preservation in Early Rectal Cancer 🩺 When pathology reveals high-risk features (pT1 high-risk or low-risk pT2) after local excision, radical TME surgery has long been considered mandatory. However, radical surgery carries significant functional costs and high permanent stoma rates. The Phase III TESAR Trial evaluated whether mesorectum-targeted adjuvant chemoradiotherapy (CRT) can safely spare the rectum: 🔹 Slide 1 — The Clinical Dilemma & Trial Design: Patients with high-risk pT1/low-risk pT2 rectal cancer after local excision were randomized (1:1) to either Adjuvant CRT (45 Gy + Capecitabine) or Completion TME surgery. 🔹 Slide 2 — The 3-Year Results: • Overall Survival: 98.9% vs. 98.9% (Equivalent oncologic safety) • Permanent Stoma Rate: 2.6% (CRT) vs. 45.4% (TME) — an over 17-fold reduction in permanent stoma risk • Locoregional Recurrence: 5.0% (CRT) vs. 1.1% (TME) • Salvageability: 80% (4 of 5 local recurrences in the CRT arm were successfully salvaged with surgery), keeping unsalvageable recurrence at just 1.3%. The Bottom Line: Adjuvant CRT is an effective, evidence-backed organ-sparing alternative for patients who prioritize sphincter preservation or face elevated surgical risk. 📚 Citation: The Lancet Gastroenterology & Hepatology. "Adjuvant chemoradiotherapy versus completion total mesorectal excision after local excision for early rectal cancer (TESAR): a multicentre, randomised, controlled, phase 3, non-inferiority trial."
