My (devil's advocate) perspective: Henderson-Hasselbach & Stewart are equivalently accurate for clinical diagnosis & management of acid-base abnormalities. Stewart is fine, but it causes people to speak a different language that leads to confusion (most clinicians use H-H). People mess up acid-base because they aren't paying attention (e.g., ignoring an elevated anion gap or a low bicarb). I'm always surprised by how a potassium of 3.4 mM causes alarm, but a bicarb of 14 mM is often ignored. Transitioning from H-H to Stewart will make you seem fancy and smart, but it won't improve your clinical acumen. If anything, it will slow you down. The best approach to clinical acid-base is a simple model that is widely utilized (H-H) and easy to apply broadly and uniformly (e.g., this approach emcrit.org/ibcc/agma/).
Part 2 of the acid–base series will be out tomorrow on thedependentvariable.com This one is entirely about the chemistry behind Stewart: strong ions, weak acids, SID, electroneutrality, where the H⁺ comes from, and what “independent” and “dependent” variables actually mean. I wanted to take the time to explain the bits that are usually skipped over. Part 3 will then bring it all back to the bedside.

Our new study is just published! We identified an association in >300 people, then followed up in mice and cells. Our findings point to a new potential hormonal cascade whereby high salt intake increases cortisol, which may suppress FGF23. Link: doi.org/10.1007/s00424-026-0…

Klotho, the "longevity protein," has long been a black box — we knew kidneys produced it, but not how it worked. #CityUHK Professor Ganesh Pathare, in collaboration with Professor Johannes Loffing, from the University of Zurich, have finally mapped its precise kidney functions - cracking the code! 🔑 Two game-changing discoveries: 🌟 Distal tubule Klotho → regulates calcium & bone health 🌟 Proximal tubule Klotho → maintains phosphate & circulating sKlotho levels Overturning long-held assumptions, this study opens new doors for treating chronic kidney disease! 🩺 🔗 Read more: go.cityu.hk/tlivyt
