Last weekend I was at @PCRSUK with @FionaMosgrove and @paul_mccallion taking bronchiectasis . So important to link primary and secondary care ❤️

I’m very proud to have been involved in writing this article - an important summary of what good care for patients affected by bronchiectasis looks like, for primary care colleagues.

I really feel this. The lack of respect for us as professionals shown as work is delegated to us - “GP to refer” etc
Soul destroying

Crazy obesity guidelines for Mounjaro on the NHS, in which a patient can have
BMI> 40kg/m2
OSA
Dyslipidaemia
Hypertension and
Pre-diabetes
Started Tirzepatide privately & lost 15kg but unaffordable so got referred to Tier 3, failed eligibility for Mounjaro (no T2D) and got switched to Wegovy starting at 0.25mg!!! And regained all the weight lost
What are we doing here???
Humans and their experience of illness are too complex to be “solved” by an algorithm. Time, continuity, relationships- ther are not algorithmic.

Reflecting on home many things in healthcare could be improved by us listening to patients and understanding their experience, and how very little it is recognised in current political shenanigans that this requires our TIME and good CONTINUITY of care.
In today’s GP land bingo, computers stop working just as I’m about to call my first patient…. Happy Wednesday to me!
A privilege to be asked to speak at the @rcgp respiratory one day essentials course. Highlighting the role of primary care in bronchiectasis. 💥
I included "learn more about respiratory" on this year's appraisal PDP, so I'm pleased to have been able to attend the @rcgp Respiratory One Day Essentials course. (It's free for RCGP members!) @FionaMosgrove has achieved the impossible and demystified bronchiectasis!

Apologizing for crying or telling someone to stop crying blocks our brain from releasing painful events.

Can we please, please stop telling chronically breathless patients who have gained weight that they “just need to lose some weight”, as if that’s the sole cause of their problem, and somehow a really easy thing for them to do?

NHS GP reaches the people at greatest need.
NHS GP is massively under resourced.
Many GPs can’t get NHS work.
My prediction: steady increase in people with enough $ go private and poorest left with a patchwork of discontinuous protocol based care with massive gaps.
Why are more GPs opting to work outside the NHS? - BBC News
