Going down the rabbit hole on hub paradox/penalty. Yikes. Airlines taking a page out of the healthcare insurance playbook.

Repairing a tracheoesophageal fistula (TEF) starts with a carefully coordinated plan. At the Baylor College of Medicine Lung Institute, that plan brings together cancer assessment, nutritional support, precise airway mapping, and vascularized tissue repair—with staged reconstruction when needed. From the planning whiteboard to the operating room, multidisciplinary teamwork guides every step toward protecting the airway and restoring swallowing. @bcmhouston @ShandaBlackmon #ThoracicSurgery #TEF #TracheoEsophagealFistula #EsophagealSurgery #BaylorMedicine

These 2 images define the problem in health care despite the noise Physicians do more work and more administrative work for significantly less compensation. Hospitals charge more and more for the services they provide But these yahoos including ‘physician leaders’ keep telling you physicians are the problems

It is so good to know that even the @Pontifex had a minimally, invasive lung cancer resected!
🫁 We wish @Pontifex Pope Leo XIV a smooth recovery. On October 13, the Vatican confirmed that he underwent #VAT surgery to remove the lower lobe of his right lung for adenocarcinoma. 🫁FACTS: ⚪️ Lung cancer can affect people who have never smoked ⚪️ Symptoms may be absent early on. A persistent cough, coughing up blood, shortness of breath, chest pain, or unexplained weight loss should prompt medical evaluation ⚪️ Screening can save lives. Ask your clinician whether you qualify for annual low-dose CT screening based on your age and smoking history. Symptoms need evaluation whether or not you qualify for screening. (cancer.gov, cdc.gov) ⚪️ There is reason for hope. Surgery can offer a chance for cure in selected patients with early-stage lung cancer (cancer.gov) At Baylor College of Medicine Lung Institute, we encourage you to know your risk, speak up about symptoms, and follow through on recommended lung nodule evaluations. #LungCancerAwareness #EarlyDetection #LungCancerScreening #BaylorCollegeOfMedicine #BCMLungInstitute

This is why Republicans keep losing the healthcare argument. Shit takes from people who don’t understand the first thing about how healthcare is actually delivered. A patient walks into a physician’s office. The physician evaluates the problem. The physician determines a procedure is necessary. The physician can safely treat the patient right there, that day, in the lowest-cost setting. And Buzz Patterson calls that a “healthcare rip-off.” Seriously? What would Dr. Oz and Buzz the healthcare expert prefer? Yes, let's send Grandma home? Tell her to come back next week? Schedule another appointment? Another drive. Another train ride. Another copay. Another day off work for her daughter. Another opportunity for her condition to get worse. Solid strategy. Medicare already has longstanding multiple-procedure rules where, in certain surgical circumstances, the highest-valued procedure is paid at 100% and additional procedures are paid at reduced rates. That is not what we are talking about here. CMS is now proposing to apply that logic when a physician provides a separately identifiable E/M visit and a procedure on the same day. The most expensive service gets paid at 100%. The other gets cut to 50%. Centers for Medicare & Medicaid Services Think about what that means. The patient comes in with a problem. The physician evaluates her. Finds the problem. Can safely fix it immediately. And CMS says: great efficiency, we’ll pay you less. That is backwards. If the physician sends her home and brings her back later, we have preserved two separate encounters. If the physician solves the problem today, CMS says there is “overlap” and proposes reducing payment. And somehow The great wizard of Oz and his sidekick Buzz have turned that into physicians “double billing.” It isn’t double billing! These are separately identifiable services. CMS itself describes the policy as an attempt to account for what it believes are efficiencies and potentially duplicative resource valuation when an E/M and global procedure occur together. Fine. Debate the valuation. Stop pretending a physician evaluated a patient twice! And while we are suddenly so concerned about waste, perhaps Dr. Oz and Buzz could look at the gigantic payment distortion sitting directly in front of him: site of service. MedPAC has been documenting this for years. Its own 2024 example: Removal of skin lesions in a clinician office: $165 Same service category in a hospital outpatient department: $404 Same patient. Same basic service. Different sign on the building. $238 more. So let me understand the healthcare reform strategy: Physician evaluates you and treats you safely in the office on the same day: Cut one of the services 50%. Move care into a hospital outpatient department: Medicare may pay dramatically more. And these guys think they found the rip-off. Incredible. This is exactly how we create incentives for care to migrate into higher-cost settings. The independent physician who can diagnose and treat the patient in one visit gets squeezed. The hospital outpatient department carries a much richer payment structure. Then Washington wonders why independent practices disappear, insurance premiums rise, and everything ends up owned by a tax-payer subsidized hospital system. @DrOzCMS, this is the healthcare problem sitting directly in front of you. Fix site of service. Stop financially rewarding care simply because it happens inside an HOPD. Reward physicians who can safely diagnose and treat patients in their offices. Reward lower-acuity care. Reward efficiency. Reward the doctor who can fix the patient in one damn visit. Don’t punish it. FYI, if you'd have called 10 independent practices, they would have nicely told you this in 60 seconds. But you retards always think you know better. Stop harming patients!

How do we determine whether a patient is healthy enough for lung cancer surgery? Our new Baylor College of Medicine Lung Institute Pulmonary Function Assessment Algorithm provides a stepwise, evidence-based approach to selecting the safest and most effective treatment. Key components include: ✅ Pulmonary function testing (FEV₁ & DLCO) ✅ Predicted postoperative (ppoFEV₁/ppoDLCO) calculations using V/Q perfusion or segment-based methods ✅ Selection of lobectomy, segmentectomy, or wedge resection based on pulmonary reserve ✅ Strategies to reduce postoperative air leaks in patients with severe emphysema ✅ When surgery is not the safest option, multidisciplinary evaluation—including Radiation Oncology for stereotactic body radiation therapy (SBRT)—ensures patients still receive potentially curative treatment. The best lung cancer care isn’t about performing the biggest operation—it’s about choosing the right treatment for the right patient through thoughtful multidisciplinary collaboration. @ShandaBlackmon explains… #LungCancer #ThoracicSurgery #PulmonaryFunction #LungCancerScreening #SBRT #RadiationOncology #MultidisciplinaryCare #CardiothoracicSurgery #BaylorCollegeOfMedicine #BCMLungInstitute #LungHealth #PrecisionMedicine #Surgery #CancerCare #ThoracicOncology

A tracheoesophageal fistula (#TEF) after #esophagectomy is a devastating #complication—but a path to recovery is possible. At the @bcmhouston Baylor College of Medicine Lung Institute, planning centers on controlling infection, repairing the airway, restoring nutrition, and rebuilding GI continuity. Staged diversion and specialized jejunal reconstruction can offer options when the stomach can no longer serve as a conduit. Complex #reconstruction takes a coordinated team—and careful protection of existing coronary bypass grafts. @BCMLungInst @ShandaBlackmon #ThoracicSurgery #EsophagealSurgery #SurgicalInnovation #ComplexEsophagealReconstruction

I need to make a very PUBLIC announcement on behalf of younger GenXers and older Millennials. If you see someone in their 40s or 50s who seems a little boring now, I need you to understand something... We already got it in. Like...GOT. IT. IN. We are not sitting home on a Friday night because we never knew how to have fun. We are sitting home on a Friday night because we have seen enough. We went to parties in houses that should have been condemned and were almost certainly owned by the local drug dealer. We saw police lights and RAN and not across a nice, flat field. INTO THE WOODS. Do you understand the athleticism required to hurdle uprooted tree roots in platform shoes while simultaneously trying to keep your low-rise jeans from sliding completely off your ass? And somehow...we never spilled the Zima. We never spilled the Boone’s Farm. That was TALENT. We slept on floors in strange houses because the designated driver either never showed up or, in a shocking turn of events, was drunker than everybody else. There was no Uber, Life360 or Find My Friends. There was just somebody named Mike saying, “I know a shortcut.” And we FOLLOWED MIKE. We closed down bars and then decided the night was still young, so we drove to Atlantic City, where we somehow managed to get drunk, sober up, get drunk again, sober up again and then get drunk a THIRD TIME before breakfast. And according to our parents? We were at Michelle’s house. Eating pizza and watching movies. Michelle’s mother unknowingly covered for approximately 17 people every weekend. If there even was a Michelle. We drank things called Purple Passion, Mad Dog 20/20 and whatever somebody mixed inside a giant plastic garbage can. Nobody asked what was in it. You grabbed a red Solo cup and trusted the process. We went to clubs until the lights came on. Do you know the psychological damage of seeing a nightclub with the LIGHTS ON? Nothing good happens after those lights come on. We danced for four straight hours in shoes that permanently altered the structure of our feet. We made entire plans without cell phones...no texts, dropped pins or group chats...just an extremely questionable sense of direction. We rode in cars with 11 people when the vehicle legally seated five. Someone was always sitting on someone. Someone was always in the hatchback. And there was always one friend yelling, “DUCK!” when we passed a cop. So yes...now we like being home. We like sweatpants. We like cancelling plans. We like saying, “That sounds fun!” with absolutely no intention of attending. We like restaurants at 5:30 because we can get a good table. We like being home by 8:17. And if someone texts at 9:30 PM and says, “What are you doing?” Sleeping...we are sleeping. Because this is not boring my friends...this is retirement. We already played, partied and danced on the bar. We already ran from the cops through a wooded area dressed like a Spice Girl. We have stories that cannot be posted because there are still people with careers, marriages and reputations to protect. And thank GOD there were no camera phones. So kids, when you look at your Gen X and older Millennial parents sitting peacefully on the couch watching Dateline at 8 PM and think: “God, you guys are boring.” Sweetheart...oh sweetheart. You are looking at retired legends. Put some respect on our names. We're probably the reason Life360 was invented. xoxoxo (posted from someone else on FakeBook) 🤣 #genx #oldandtired
