Awesome results. Couldn’t agree more re: adrenaline (apart from calling it epinephrine). Lots of talk about ECMO for rVF but adrenaline is a low-hanging fruit 🍇
Our Cardiac Arrest protocol for VF at @PBCFR does not follow standard AHA guidelines. Here's what we do: 1. HP-CPR 2. Early defibrillation 3. No Epinephrine 4. DSED for rVF 5. Esmolol for rVF YTD Arrests Worked: 524 Overall Survival to D/C: - PBCFR: 13.2% - National: 7.5% Utstein Survival YTD via @CARESRegistry: - PBCFR: 41.7% - National: 23.9% Utstein Bystander Survival YTD: - PBCFR: 38.5% - National: 27.2% #Epinephrine could (and should) be #removed from the shockable rhythm protocol.

You see this, this is what happens in real life. The bots can't show up on the streets. On the right there (yeah the far right) are the 6 nazis who turned up to cause mayhem. Everyone else is there to stop them. This is England.

This is something I teach on every #SPEAR course but yet often struggle to actually describe the technique, this thread explains it perfectly!
🧵regarding the useful technique of 'straightening the wire' during central line placement when the plastic guide is no longer within reach. If you do a lot of central lines (or a lot of percutaneous procedures), this technique will come in handy over and over again. (1/ )

Paramedics: Does your ambulance service do POC ketone testing? Aside from DKA, Are you also testing for euglycaemic ketoacidosis in unwell patients taking SLGT2 inhibitors ? (Often times these patients don’t have diabetes- they have HF) (We probably should be checking)

Study categorisation from @pocock_stuart at #PratoAnaes2022 ➡️ WOW! and PHEW! interpreted the same but very different ➡️ PHEW! and DAMN! interpreted differently but virtually the same We need to get wiser, a p-value is no substitute for brain.

He brazenly lied to @covidinquiryuk about "losing" all those WhatsApps. Now he's brazenly lied on national TV. He's Johnson, basically, with a glossy £700 million veneer. I'm so utterly sick of these Trump-lite, devoid-of-integrity, grubby little political charlatans.

The "homemade limoncello" video is up there with the very best of the internet. 4 years later and I still laugh at it on every play.

Welcome to the new UK vs US medical debate: “MidaKet” vs “Ketolam”
New blog: MidaKet for procedural sedation in critical illness ⚖️ Midazolam & ketamine yield synergistic sedation while minimizing doses & side-effects from either agent alone (multimodal sedoanalgesia). ⚖️ Avoids suppressing respiration or Bp. post: emcrit.org/pulmcrit/midaket/

Unless one operates in a region where opioid overdoses are truly endemic I just don’t think having law enforcement carry naloxone is a good idea. If a member of the public isn’t breathing they need CPR Not repeated doses of naloxone and many people standing around waiting

I have recently come across a challenging case where it felt like everything fell into place and we had a good outcome. In debrief a common theme that came out that the treatment of such an extreme case are not in the guidelines Hence a 🧵 on

Scope creep
Every so often, I am reminded of how Edwardian England was completely consumed by the horror of WWI. One such moment today at the Singer-Sargent exhibition. Lady Helen Vincent as confected gorgeousness. She became a nurse and anaesthetist in the war.
