Thank you to @tompowles1 for asking the question and to The @Uromigos and @shilpaonc for this very important conversation about Upper Tract Urothelial Carcinoma. Yes to a trial and yes to well-considered options for UTUC patients while we wait! It does my heart good to see this complex disease get continued attention post POUT. creators.spotify.com/pod/pro…
It looks like we’re not sure about the best systemic approach to upper tract urothelial cancer. Data for nivolumab in Upper tract disease is not great (DFS HR- 1.23 (0.67-2.23), while the carboplatin subset of POUT was also underwhelming (OS HR 0.87: 0.50-1.53). ctDNA appeared to work for UTUC in IM010 but was excluded in IM011 due to concerns from the nivo data in this subgroup. EVP has always beaten everything else in UC, and upper tract subsets of 302 looked good - but there in no adjuvant data. What to do? I’m not sure. In ctDNA positives I might be tempted to go with EVP and survey the -ves (and give EVP if they relapse), but it feel a long way off piste! @MattGalsky @OncoAlert @montypal @DrChoueiri @AlisonBirtle

⚡️ Phase 2 trial in RCC with inferior vena cava tumor thrombus: neoadjuvant stereotactic ablative radiotherapy (SAbR) followed by surgery (n=23). 1-year recurrence-free survival: 71.4% — above the 55.7% historical benchmark. Thrombus downstaging in 22%. A promising approach for one of the most challenging complications in kidney cancer. #KidneyCancer #RCC @JAMAOnc jamanetwork.com/journals/jam…

🩺 The evolving role of SBRT in metastatic kidney cancer At the 2nd Florence International Consensus, Prof. Chad Tang from @UTMDAnderson highlights how stereotactic body radiotherapy (SBRT) is becoming an increasingly important part of metastatic renal cell carcinoma (RCC) management. #FlorenceConsensus26 🎯 Oligometastatic disease SBRT may help defer systemic therapy and its associated side effects in selected patients. Local treatment approaches—including radiotherapy, surgery and interventional ablation—offer options that work through mechanisms distinct from systemic therapy. 🔬 Oligoprogressive disease By targeting a limited number of progressing lesions, SBRT may allow patients to remain on their current systemic treatment for longer, potentially delaying the need to switch to subsequent lines of therapy. 💡 The key takeaway: Across the metastatic disease spectrum, SBRT is expanding the role of local treatment in kidney cancer care, supporting more tailored treatment strategies and helping clinicians consider how best to manage disease progression while preserving treatment options. 📍 As evidence and clinical experience continue to evolve, multidisciplinary decision-making remains essential to identifying the patients most likely to benefit. #KidneyCancer @Lucarecco @CarlottaB @RobClarkeLab @JavierCortesMD @curijoey @delmastro_lucia @ALEDIGNUS12 @Prof_Nadia_H @ClareIsacke @reshmajagsi @KaidarOrit @AnnaKirby17 @matteolambe @Icro_Meattini @hoperugo @TanjaSpanic @MassimoDiMaio75 @Silke_Gillessen @DrRanaMcKay @brian_rini @alison_tree @ChadTangMD @DrYukselUrun @MicheleAquilano @GiulioFrancoli1 @DrDanielHeng

🟢 Precision Oncology through Biomarkers: Assessing What Matters Most — Renal Stefanie Zschäbitz at #IUCS26 🟢 ctDNA is emerging as a powerful prognostic tool in RCC, but its role still depends strongly on disease setting, assay sensitivity and timing. 🟢 In the perioperative setting, tumor-informed ctDNA may help identify patients at higher risk of recurrence, while serial pre- and post-surgical assessment appears more informative than a single time point. 🟢 In KEYNOTE-564, ctDNA positivity identified a subgroup with markedly poorer disease-free survival — reinforcing the potential of molecular residual disease as a future risk-stratification tool. 🟢 KIM-1 is another promising biomarker in RCC: a transmembrane glycoprotein overexpressed in clear-cell and papillary RCC, measurable both in tissue and serum, with growing interest in prognosis and treatment monitoring. 🟢 PD-L1 alone remains insufficient as a treatment-selection biomarker in mRCC. Across contemporary immune-based combinations, benefit cannot be reliably reduced to PD-L1 status alone. 🟢 Ongoing strategies such as CARE-1 are testing whether biomarker-defined populations can better guide the choice between dual immunotherapy and immunotherapy + anti-angiogenic therapy. 🟢 Belzutifan illustrates precision oncology beyond conventional biomarkers, with established activity in VHL-driven disease and growing real-world and trial evidence across RCC settings. 🟢 Emerging targets are expanding the renal cancer biomarker landscape: • FAP as a marker of aggressive biology and potential imaging target • Carbonic anhydrase IX for theranostic approaches • ROR2, CAIX and CD70 as potential cellular-therapy targets 🟢 The take-home message: precision oncology in RCC is moving from static single biomarkers toward dynamic, multimodal assessment — tissue, ctDNA, serum markers, imaging and tumor biology — used at the right time for the right clinical question. @neerajaiims @gbanna74 @Sebastiano_Buti @CinziaConted @dralishadamani @nataliagandur @amg120395 @PGrivasMDPhD @ravikanesvaran @koshkin85 @MicheleMaffezz @ManiamAkash @ParamMariappan @dmukherji @VanitaNoronha @montypal @DanieleRaggi83 @PasRescigno @jteoh_hk @alison_tree @DrYukselUrun @yekeduz_emre @myESMO @ASCO @ASCOPost @AmerUrological @urotoday @UrologyTimes @OncLive @ecancer @Uroweb #KidneyCancer #RCC #GUOncology #PrecisionOncology #Biomarkers
