After both ECTRIMS2026 and American Neurological Association, I was reflecting on how #autoantibody-mediated neurological diseases are such precise clinical and biological entities. Few other diseases carry such homogeneity.
In 2023, @EoinFlanagan14 @adrianbudhram1 and I proposed it would be better to split, rather than lump. We suggested that the antibody itself - especially when highly specific - should be the gateway to diagnosis.
The fundamental demographics, clinical features and biological characteristics differ so markedly (see table) based on the antigen
It’s a key theme across our two main 2026 reviews and calls for a rethink re nomenclature of these conditions.

The autoantigen defines so so so much! It must be put at the center.
Many congratulations to @joshmalapit today. Josh won the Dean of Research Retreat poster prize @MayoClinic Florida for his work on neuromyelitis optica - and the origins of AQP4 autoantibody production. He is a fantastic human being and is currently putting together the paper with @darshi_r @antibodypad and others ……so I won’t disclose the super exciting punchline 🥳.
Thank you @anaselgenidy1
The session was great with several discussions around immune reset, naive vs memory B cell roles post immunotherapy and CAR-T therapy.
Excellent presentation by Dr. Irani @Mayo_AING at #ANA2026 on a fundamental question in neuroimmunology: What causes autoimmunity in humans?
Fascinating insights into how central and peripheral immune tolerance shape CASPR2-directed autoimmunity.

Truly a team effort with
@burcuzeydanmd @albertaboseif @ANASTASIA_ZEK @EoinFlanagan14 @Mayo_AING and colleagues! 🧠👏
Supportive of that view but……isn’t it amazing that only these #LGI1 #antibodies produce this phenomenon? So so so specific. The question is why. That’s the real dilemma to solve
This commentary proposes that faciobrachial dystonic seizures may reflect an immune-mediated motor network mechanism involving both cortical and striatal dysfunction.

Great work Matteo
CASPR2 and LGI1 antibodies FUNCTION, rather than titre, could be a useful prognostic biomarker in autoimmune encephalitis. Congrats to @businaro_pietro for the amazing work! neurology.org/doi/10.1212/NX…

100% agree. For me, despite the hatch for BF, Kobbie was today’s best player. His simple touches so often get us out of trouble or quickly into a new play. Brilliant player!
🚨🗣️ Wayne Rooney: “Kobbie Mainoo? I thought he was miles ABOVE everyone: his decision making, his calmness on the ball.
“Knowing when to inject a few yards of space to get past a player. I thought he was a breath of FRESH air.
“I thought Youri Tielemans did WELL alongside him as well”. #MUFC [@UnitedStandMUFC]
A ‘trajectory’ analysis certainly matters sense!

Yes, but accounting for age-related hippocampal shrinking. (Which is the reason why I intend to leave 24 hrs before everybody starts thinking that I should leave) 😆
Makes perfect sense to me. Your actual track record is the best metric of your future work. So much relies on the PI/person: mentality, drive, strategy etc - and that tends not to change. There are exceptions but, for me, proven output is the best measure of future productivity
I’m going - would be fab to see as many other neuroimmunologists there as possible!!

The debates are new this year at the @AANmember - should we keep them?!
The speakers sure seem to be having fun!

Super impressive. Many congratulations. We’re all very proud of you.
I’m truly honoured and deeply grateful to share that I have been awarded an SNSF Ambizione Grant from the @snsf_ch. Over the next 4 years, I will investigate how neuroinflammation drives synaptic dysfunction and neurodegeneration in encephalitis.
