Six phase III trials. Six different stories.
Higher pCR does not guarantee better survival.
In muscle-invasive bladder cancer, EFS and OS matter more than impressive response rates.
Are we measuring what truly matters to patients?

With VOLGA results coming at #ESMO26, a summary of contemporary phase III perioperative trials in muscle-invasive bladder cancer.
⚠️ Different populations and designs—avoid cross-trial comparisons. VOLGA and EV-303 importantly include surgery-alone control arms without neoadjuvant therapy.
🔑 The next challenge: identifying who truly needs adjuvant ADC therapy—and who can safely avoid it.

ALT This image contains four graphs showing the key secondary end points. Graph A shows event-free survival over time in months since randomization comparing ADT plus apalutamide and ADT plus placebo. Graph B presents the first subsequent local or systemic therapy occurrence, with the same treatment comparison. Graph C illustrates distant metastasis or conventional imaging or PSMA PET. Graph D compares freedom from disease at four years with percentages for both treatment groups. Each graph includes shaded confidence intervals and hazard ratios with confidence intervals.

Durvalumab + BCG is FDA approved in high risk NMIBC , by hitting its DFS - HR 0.68 (plus a ~⬇️ cystectomy rate). OS & M1 data show these patients have low cancer mortality & IO has about a 10% chance of serious side effects. Therefore, this is not a treatment for all HR NMIBC IMO. It’s hard to know who to select. Inconsistency with other data for Sasanlimab + atezo means there is no subset that consistently benefits. It will be interesting to see whom urologists recommend for treatment. #ASCO26 astrazeneca.com/content/astr…

Aspirin & cancer prevention—myth or fact? 🤔
A @JAMAOnc paper👉
doi:10.1001/jamaoncol.2025.6196
In adults ≥70, 4.7 yrs of low-dose aspirin didn’t lower cancer risk and may increase cancer-related deaths during treatment. No long-term benefit seen.

Visibilizar = avanzar 💙

1/🚨 First-line breakthrough in la/mUC HER2+ 🇨🇳
DV + toripalimab significantly improves PFS (HR 0.36) and OS (HR 0.54) vs chemo.
Striking efficacy across subgroups, including HER2 IHC1+, with a favorable safety profile.
🔜 A new SOC?
