PR after induction - "Go on" to ASCT OR "Hold on" for VGPR and then do ASCT. A dilemma which many of us face in practice. Our analysis from my previous workplace which answers this important question @ishbtish @IndMyAcGp @DrPMPGI @UdayYanamandra @IsbmtU

I like this approach. Depositing light chains is toxic, and the treatment of AL should be considered a quasi-emergency! Almost like renal failure in MM. #everydaymatters

Important clinical pearl from @Ron_Witteles! In cardiac AL, I check FLCs q-weekly in the first couple of months, and switch treatment if I sense plateau. “Every day matters” for the component of toxic cardiomyopathy from circulating FLCs.
Congrats to the authors, but 2 critical points: 1) There’s no role for PYP scan in setting of + monoclonal protein. 2) “Promptly diagnosing” does not mean 1-2 months from time of suspicion. When #AL #amyloidosis is in the differential, it’s a near-emergency — every day matters.

Went to pick up a prescription at the pharmacy and with my prescription card it was $467…. Cash price $63.40. Our healthcare and insurance system, and the Pharmacy benefit managers have created the biggest fraud machine I’ve ever seen.

Google just announced a breakthrough in quantum computing. It's called "Willow" - and it's making waves in the tech world. But buried in their announcement was something fascinating: A discovery that could reshape our understanding of computing itself...

Most importantly: MRI accounts for a significant number of myeloma-defining events (MDE) and proofs to be a valuable tool.

We will be publishing long-term follow up of various total therapies. We presented last year @ASCO on sum of the long term results ascopubs.org/doi/10.1200/JCO… Approximately one out of three patients (standard risk) are cured using such approach 20-yr OS is ~25% in TT II 🌟

The problem with the analysis you describe is that mrd measured at 1 year after the transplant decision made. 2/3 of mrd neg patients were in the transplant arm. Don’t get confused by the data. Transplant offers huge benefit
Agreed with @GvHD_Meade - to me, the most fascinating part of DETERMINATION was that patients who achieved MRD negativity with just VRd had no real benefit to ASCT. Now that we have even better quads, MRD-guided studies like MASTER-2 (cc @End_myeloma) will show us the future!

100%. It’s always fascinated me that the myeloma field is all about intensifying therapy to the utmost - with the *sole* exception of autoHCT, which is the only case where de-escalation seems to be a priority. Yet autoHCT is the cheapest and (arguably) most effective tx we have.

I have a better one for you. Diagnosed in 2006.light chains jumped up/down between 700 to 1000 mg/dL (NOT MG/L). Not sure if real or assay issues but still “high”. Would def. Qualify for therapy based on FLC ratio. No crab. 10-15% plasma cell in the marrow.11;14. 1/
