Reflecting on the key themes of #IMS2026 - >5 yr remission for newly diagnosed and >3 yrs in relapse for most MM is becoming the standard - High Risk MM is still a challenge, immunotherapy is key. Paucity of data in EMD and PCL. Agree with @VincentRK in adding PCL to HR definition -Access is key- after meeting with several drug manufacturers, this is being thought of in the design, clinical trial, execution both for CART and BsAb. Can every patient get this within 60 miles of their home. - TRM is more important esp. in the first 6 months -Less is more- Newer CART's have lesser CRS, ICANS and newer BsAb's have less intense dosing schedule with lower CRS -Dual Ag targeting is A way forward, a/w how it compares against good single agent targets -There is a paucity and some progress in understanding rare toxicities- Colitis, cerebellar syndromes, oral toxicities, rare infections -if we are going to give subsequent immunotherapies, we need to be more precise, with sequencing and T cell fitness, both of which are still in its infancy in MM -Except celmods there are no other non targeted therapies [non BCMA, GPRC5, FCRH5]. Is this enough ? [I am aware of others but none in Ph 3] -Cure - covered by @Mohty_EBMT already Thx for the organizers @profghjackson @NikhilMunshiMD @NoopurRajeMD @IMFmyeloma for a wonderful meeting and looking forward to next year

First RCT results of Etentamig (BCMA bispecifc antibody) presented at Plenary Session #IMS26 @Myeloma_Society @PlasmaCellPete @myelomaMD 60 mg fixed dose every 4 weeks makes this the easiest bispecific to administer. Impressive PFS benefit in relapsed refractory myeloma with at least 2 prior lines of therapy. See thread for more details.

Now online @NatureComms, our manuscript describing why the myeloma CAR T cell cilta-cel is more effective than a very similar CAR, ide-cel. Led by the very talented Kevin Herrera and Sneha Mitra, and in collaboration with the Leslie Laboratory. @MSKCancerCenter @szusmani

🚨 Modest @BloodPortfolio title doesn't do justice to TRIMM-2 analysis. Tec-dara in CD38-refractory myeloma (as long as ≥90-day washout): ORR 68%, mPFS 17 months (longer than TEC-1) Does tec-dara work in CD38-refractory myeloma? Evidently, yes! Congrats @paurotero et al 👏
#Myeloma Paper of the Day: TRIMM-2 study of teclistamab & daratumumab in RRMM finds ORR 68.9% (CR or better 44.3%); median DOR not reached; median PFS 26.3 mos; most common TEAEs = infxns, CRS, neutropenia, anemia; grade 3/4 TEAEs in 93.4%, 7 died: pubmed.ncbi.nlm.nih.gov/4268…. #mmsm

The @FDA has approved daraxonrasib for adult patients with previously treated metastatic pancreatic cancer. The approval is based primarily on results from the Dana-Farber-led RASolute 302 trial showing daraxonrasib nearly doubled median overall survival versus chemotherapy. Brilliantly said! Thankfully trials often better about not excluding "history of" PCL, but not always. Non-bone-based EMD not fundamentally viewed to be different disease; neither should PCL. @MyelomaOslo et al working on IMWG guidelines for trial eligibility to address this!
Just out! Our ViewPoint in @TheLancetHaem We call for plasma cell leukemia to be considered as high risk myeloma and not as a unique disease. Free Open access Link for limited time. Bookmark. nam12.safelinks.protection.o…

Another nice bsAb myeloma analysis from Fogel @Phhersh @BiranNoa et al. Only two predictors of OS in MVA: 1️⃣ Infections (HR 2.14, 95% CI 1.1-4.3) 2️⃣ Using IVIG (HR 0.28, 95% CI 0.1-0.7) Curves (albeit non-landmarked) = 1000 words. Don't wait for infections: start IVIG now!

Iberdomide (first CELMoD) now @US_FDA ✅ based off EXCALIBER-RRMM in 2L and beyond for R/R #MultipleMyeloma. IberDd: - Improved MRD (41% vs. 21%) - PFS still immature - AEs: Cytopenias and Infections #OncTwitter #MedTwitter #HemeTwitter

Love seeing triple-digit MRD neg percentages 🤩 Congrats @RaabMarc et al on excellent MajesTEC-5 @NatureMedicine paper! bsAb-based induction in ND myeloma - may well obviate ASCT. Top panel = ITT, bottom = MRD-evaluable. Kudos to authors for reporting both sets like this.
#Myeloma Paper of the Day: Teclistamab-based induction in TE NDMM shows MRD- CR rate 91.8% (45/49) by pre-maintenance timepoint, 100% in evaluable samples & post-induction cycle 3 (10-5 (46/46)), 6 (10-5 (46/46) & 10-6 (46/46)); ORR was 100%: pubmed.ncbi.nlm.nih.gov/4235…. #mmsm

History in action here at #EHA2026! Such impressive PFS & OS with a BCMA-sparing #downwithdex regimen in myeloma... a few of my own patients are on these K-M curves and doing great. Tal-Dara ± pom (adding pom: more neutropenia/fatigue but ❓ slightly higher PFS) coming soon!

1/ Our call to action is out in @BloodAdvances! Title speaks for itself. Given concerningly high infection risk with bsAbs in myeloma #MMsm, IVIG/SCIG should be started regardless of any IgG threshold. Global authors & global audience, including insurance & healthcare funders.
